应用超声联合钼靶对乳腺良恶性肿块的对比分析

摘要 目的探讨乳腺超声、钼靶及两者联合检查对诊断乳腺良恶性肿块的临床价值。方法选取行超声、钼靶及两者联合检查的患者共115人(128个乳腺肿块),采用2013版BI-RADS分级对肿块进行分类,以病理结果为金标准,绘制受试者工作曲线,计算其敏感度、特异度及曲线下面积。计算超声、钼靶及联合检查与病理结果的一致性。结果128个肿块,良性40个,恶性88个。乳腺超声诊断的敏感度、特异度、准确度分别为84.09%,87.50%,85.16%;乳腺钼靶诊断的敏感度、特异度、准确度分别为81.82%,87.50%,83.59%。二者联合检查的敏感度、特异度、准确度分别为92.05%,82.50%,89.06%。超声、钼靶、联合检查的ROC曲线下面积分别为0.858、0.847、0.873。超声、钼靶、联合检查与病理结果的Kappa值>0.6,与病理结果均具有较强一致性。结论超声与钼靶诊断乳腺肿块均与病理结果具有高度一致性,具有较高准确性,两者联合检查比单项检查具有更高的准确性,值得临床推广应用。 Objective:To investigate the comparative analysis of ultrasound,mammography and their combined examination in the diagnosis of benign and malignant breast tumors.Method:115 patients of breast toumor(128 toumors)were selected for Pathological biopsy.Breast imaging reporting and data system(BI-RADS),was used to classify 128 breast.With pathological results as the gold standard,pathological results as the gold standard,ROC curve was used to assess the sensitivity,specificity and the area under with pathological results was calculated.Results:128 breast tumors,40 were benign,88 were malignant.The sensitivity,specificity and accuracy of Ultrasound were 84.09%,87.50%and 85.16%,the sensitivity,specificity and accuracy of mammography were 81.82%,87.50%and 83.59%,The sensitivity,specificity and accuracy of combined examination were 92.05%,82.50%and 89.06%,respectively.Consistency of ultrasound,mammography and combined examination with pathological results,Kappa value>0.6,showed a strong consistency with pathological results.[Conclusion]Both ultrasound and mammography have high accuracy in the diagnosis of benign and malignant breast tumors,and are highly consistent with the pathological results.The combined examination of ultrasound and mammography has higher accuracy than the single examination,and is worthy of clinical application.
