摘要
目的探究阿柏西普联合全视网膜激光光凝术治疗糖尿病视网膜病变的临床效果。方法66例糖尿病视网膜病变患者,随机分成对照组和观察组,每组33例。对照组患者采用全视网膜激光光凝术治疗,观察组患者采用阿柏西普联合全视网膜激光光凝术治疗。比较两组患者视力改善效果、治疗相关并发症发生情况及治疗前后黄斑中心凹视网膜厚度、最佳矫正视力、眼压、眼底相关指标。结果激光治疗结束后3个月,观察组视力改善总有效率为96.97%(32/33),高于对照组的81.82%(27/33),差异具有统计学意义(P<0.05)。激光治疗后3个月,两组患者黄斑中心凹视网膜厚度低于本组治疗前,最佳矫正视力高于本组治疗前,且观察组患者黄斑中心凹视网膜厚度(227.06±23.08)μm明显低于对照组的(263.00±29.55)μm,最佳矫正视力(0.54±0.10)显著高于对照组的(0.47±0.12),差异具有统计学意义(P<0.05)。激光治疗后3个月,两组患者眼底渗出灶面积、出血灶面积均小于本组治疗前,微血管瘤数目少于本组治疗前,差异具有统计学意义(P<0.05)。观察组患者治疗相关并发症发生率3.03%(1/33)与对照组的6.06%(2/33)比较,差异无统计学意义(P>0.05)。结论阿柏西普联合全视网膜激光光凝术治疗糖尿病视网膜病变效果显著,在改善患者视力、减轻黄斑水肿方面更具优势,且不会增加并发症,值得推广应用。
Objective To investigate the clinical effect of aflibercept combined with panretinal laser photocoagulation in the treatment of diabetic retinopathy.Methods A total of 66 patients with diabetic retinopathy were randomly divided into control group and observation group,with 33 cases in each group.The control group was treated with panretinal laser photocoagulation,and the observation group was treated with aflibercept and panretinal laser photocoagulation.Both groups were compared in terms of effect of visual acuity improvement,incidence of treatment-related complications,central retinal thickness,best corrected visual acuity,intraocular pressure and fundus related indexes before and after treatment.Results At 3 months after laser treatment,the total effective rate of visual acuity improvement in the observation group was 96.97%(32/33),which was higher than that of 81.82%(27/33)in the control group,and the difference was statistically significant(P<0.05).At 3 months after laser treatment,the central retinal thickness in both groups was lower than that before treatment in this group,and the best corrected visual acuity was higher than that before treatment in this group;the central retinal thickness of(227.06±23.08)μm in the observation group was significantly lower than that of(263.00±29.55)μm in the control group;the best corrected visual acuity of(0.54±0.10)in the observation group was significantly higher than that of(0.47±0.12)in the control group;the differences were statistically significant(P<0.05).At 3 months after laser treatment,the area of fundus exudation focus and area of hemorrhage focus in both groups were lower than those before treatment in this group,and the number of microangioma was lower than that before treatment in this group.The differences were statistically significant(P<0.05).The incidence of treatment-related complications in the observation group was 3.03%(1/33),and the difference was not statistically significant compared with that of 6.06%(2/33)in the control group(P>0.05).Conc
