摘要
1例83岁男性患者,因反复咳嗽、咳痰20余年,加重5 d入院,诊断为慢性阻塞性肺疾病急性加重。入院当天给予苹果酸奈诺沙星氯化钠注射液抗感染治疗,约3 min后患者出现气憋、口唇及双手麻木、全身潮红、四肢湿冷、颜面部及下肢水肿等不适症状,患者指尖血氧饱和度下降至70%,血压降至65/39 mm Hg,考虑为药物所致过敏性休克,立即停用该药,同时给予注射用甲泼尼龙琥珀酸钠(40 mg,st,ivgtt)、盐酸肾上腺素注射液(l mg,st,iv),无创呼吸机辅助通气等对症治疗。约20 min后患者症状逐渐减轻,后未出现类似症状,于病情稳定后出院。
An 83-year-old male patient was admitted to the hospital with a diagnosis of acute exacerbation of chronic obstructive pulmonary disease(COPD)due to recurrent cough and sputum for more than 20 years with a 5-day exacerbation.On the day of admission,the patient was given naloxacin malate sodium chloride injection and developed short of breath,numbness of the lips and hands,general flushing,wet and cold extremities,facial and lower limb edema,etc about 3 minutes later.Fingertrip oxyhemoglobin saturation dropped to 70%and the blood pressure dropped to 65/39 mm Hg,which was considered as anaphylactic shock.Nemonoxacin malate and sodium chloride injection was immediately discontinued,and symptomatic treatment including methylprednisolone sodium succinate for injection(40 mg,st,ivgtt),epinephrine hydrochloride(1 mg,st,iv),and non-invasive ventilator assisted ventilation were given to him.The symptoms gradually decreased after about 20 minutes,and then he was discharged in stable condition.
