摘要
目的:探究肺腺癌采用甲状腺转录因子-1(TTF-1)、细胞角蛋白7(CK7)、天冬氨酸蛋白酶(Napsin A)进行联合检验的价值。方法:选取2021年1月—2022年11月许昌北海医院收治的非小细胞肺癌患者100例作为研究对象,根据病理诊断结果分为肺鳞癌组、肺腺癌组,各50例。使用免疫组化法,检测患者病理组织中TTF-1、CK7、Napsin A阳性表达率,分析TTF-1、CK7、Napsin A单独检验与联合检验的准确率、敏感性和特异性及有无淋巴结转移患者联合检验阳性、1项阴性情况。结果:肺腺癌组TTF-1、CK7、Napsin A阳性表达率高于鳞癌组,差异有统计学意义(P<0.001);联合检验准确率、敏感度及特异度均高于三项指标单独检验,差异有统计学意义(P<0.05);肺腺癌淋巴结转移患者联合检验阳性率高于未转移患者,差异有统计学意义(P<0.05);淋巴结有转移患者有1项阴性率低于未转移患者,差异有统计学意义(P=0.003)。结论:TTF-1、CK7、NapsinA单项检测,能够帮助患者鉴别肺腺癌,通过联合检测,可获得较高的准确率,同时,肺腺癌患者淋巴结转移者中,阳性表达率较高,可为临床诊断提供参考。
Objective:To investigate the value of the combined test using thyroid transcription factor-1(TTF-1),cytokeratin 7(CK7),and aspartic protease(Napsin A)for lung adenocarcinoma.Methods:One hundred cases of non-small cell lung cancer patients admitted to Xuchang Beihai Hospital from January 2021 to November 2022 were selected as the study subjects,and they were divided into lung squamous carcinoma group and lung adenocarcinoma group according to the pathological diagnosis,fifty cases each.Using immunohistochemistry,the positive expression rates of TTF-1,CK7,and Napsin A were detected in the pathological tissues of the patients.The accuracy,sensitivity and specificity of TTF-1,CK7,and Napsin A alone and in combination were analyzed,as well as the combined test positivity and 1 item negativity in patients with or without lymph node metastasis.Results:The positive expression rate of TTF-1,CK7,and Napsin A was higher in the lung adenocarcinoma group than in the squamous carcinoma group,and the difference was statistically significant(P<0.001);The accuracy,sensitivity and specificity of the combined test were higher than those of the three indexes individually,and the difference was statistically significant(P<0.05);The positive rate of the combined test in patients with lymph node metastasis of lung adenocarcinoma was higher than that in patients without metastasis,and the difference was statistically significant(P<0.05);The negative rate of 1 item was lower in patients with lymph node metastasis than in patients without metastasis,and the difference was statistically significant(P=0.003).Conclusion:TTF-1,CK7 and Napsin A single test can help patients to identify lung adenocarcinoma,and a higher accuracy can be obtained by combined test,meanwhile,the positive expression rate is higher in lung adenocarcinoma patients with lymph node metastasis,which can be used as a reference for clinical diagnosis.
