甲状腺微小癌患者血清TSH、TTF-1与超声造影参数相关性及联合检测价值

Correlation of serum TSH and TTF-1 with ultrasonographic parameters and value of combined detection in patients with microscopic thyroid cancer

摘要 目的探讨甲状腺微小癌患者血清促甲状腺激素(TSH)、甲状腺转录因子-1(TTF-1)与超声造影参数相关性及联合检测价值。方法前瞻性选取河北省残疾人康复中心和哈励逊国际和平医院2018年8月至2020年8月甲状腺微小癌患者106例作为恶性组,另选取同期良性甲状腺结节患者106例作为良性组。比较两组血清TSH、TTF-1、超声造影参数[造影剂峰值强度(PI)、达峰时间(TTP)、时间-强度曲线下面积(TIC-AUC)],分析血清TSH、TTF-1与超声造影参数相关性,评价血清TSH、TTF-1、超声造影参数单独及联合诊断甲状腺微小癌价值,分析血清TSH、TTF-1、超声造影参数与病理特征的关系。结果恶性组血清TSH、TTF-1高于良性组,PI、TIC-AUC低于良性组,TTP长于良性组(P<0.05);血清TSH、TTF-1与PI、TIC-AUC呈负相关,与TTP呈正相关(P<0.05);血清TSH、TTF-1联合PI、TIC-AUC、TTP诊断甲状腺微小癌的AUC为0.914(95%CI:0.868~0.948),敏感度为87.74%,特异度为84.91%,明显优于各指标单独诊断;血清TSH、TTF-1、TTP与肿瘤直径、淋巴结转移、腺外浸润呈正相关,与分化程度呈负相关,PI、TIC-AUC与肿瘤直径、淋巴结转移、腺外浸润呈负相关,与分化程度呈正相关(P<0.05)。结论甲状腺微小癌患者血清TSH、TTF-1与超声造影参数PI、AUC、TTP显著相关,联合检测可作为临床诊断的重要途径,且与多项病理特征密切相关,能为临床评估病理进展提供有效信息。 Objective To investigate the correlation between serum thyroid stimulating hormone(TSH),thyroid transcription factor-1(TTF-1)and ultrasonography parameters and the value of combined detection in patients with microscopic thyroid cancer.Methods Prospectively,106 patients with microscopic thyroid cancer from August 2018 to August 2020 in Hebei Rehabilitation Center for Disabled Persons and Harrison International Peace Hospital were selected as the malignant group,and another 106 patients with benign thyroid nodules in the same period were selected as the benign group.Serum TSH,TTF-1,and ultrasonographic parameters[peak contrast intensity(PI),time to peak(TTP),and area under the time-intensity curve(TIC-AUC)]were compared between the two groups,and the value of serum TSH,TTF-1,and ultrasonographic parameters alone and in combination for the diagnosis of microscopic thyroid cancer were evaluated.The relationship between serum TSH,TTF-1,ultrasonographic parameters and pathological features was investigated.Results In the malignant group,serum TSH and TTF-1 were higher than those in the benign group,PI and TIC-AUC were lower than those in the benign group,and TTP was longer than that in the benign group(P<0.05).Serum TSH and TTF-1 were negatively correlated with PI and TIC-AUC and positively correlated with TTP(P<0.05).Serum TSH and TTF-1 combined with PI,TIC-AUC and TTP was 0.914(95%CI:0.868-0.948),with a sensitivity of 87.74% and specificity of 84.91%,which was significantly better than each index alone in the diagnosis of microscopic thyroid cancer.PI and TIC-AUC were negatively correlated with tumor diameter,lymph node metastasis and extra-glandular infiltration positively correlated with differentiation degree(P<0.05).Conclusion Serum TSH and TTF-1 were significantly correlated with ultrasonographic parameters PI,AUC and TTP in patients with microscopic thyroid cancer.The combined test can be an important way of clinical diagnosis and closely correlated with several pathological features,which can provide effectiv
