摘要
目的:分析我院铜绿假单胞菌的分布、耐药性变迁及与其他医疗机构差异性,为临床合理用药提供依据。方法:收集2017—2021年我院各科室1551株铜绿假单胞菌阳性的临床标本,按照《全国临床检验操作规程》(第4版)的方法进行培养分离,采用VITEK2-COMPACT自动分析仪进行细菌鉴定和药敏分析,结果判定按照2016年美国临床实验室标准化协会(CLSI)判断标准进行。结果:2017—2021年共送检40882例,分离出阳性菌株9679株,所送检标本按类型来源及占比前5位的为痰液、血、尿液、脓液及创伤感染分泌物、脑脊液。其中,PA菌株1551株,所送检标本按类型来源及占比前4位的为痰液、尿液、脓液及创伤感染分泌物、血;科室分布以重症医学科、呼吸科为主,其他科室如血液肿瘤科、泌尿外科、神经外科、胸外科、其他科室分离数较少。2017—2021年MDR-PA菌株分别为6株、9株、9株、9株、11株。总耐药率只有头孢他啶超过20%,其他抗菌药物包括阿米卡星、亚胺培南、左氧氟沙星、头孢哌酮舒巴坦、环丙沙星、头孢吡肟、美罗培南和哌拉西林/他唑巴坦总耐药率均低于20%。结论:我院分离出的铜绿假单胞菌主要以痰标本为主,分布在重症医学科、呼吸科,同时铜绿假单胞菌耐药性相较于国内相关文献偏低,上述数据与国内其他医院有较明显差异性,提示基层医院细菌分布及耐药性既受医院抗菌药物管理规范化程度影响,也受当地其他医疗机构抗菌药物应用影响。
Objective:To analyze the distribution and drug resistance changes of Pseudomonas aeruginosa in our hospital and its differences from other medical institutions,so as to provide basis for clinical rational drug use.Methods:The clinical specimens of 1551 strains of Pseudomonas aeruginosa positive in various departments of our hospital from 2017 to 2021 were collected and cultured according to the method of“National Clinical Laboratory Procedures”(4th edition).The VITEK2-COMPACT automatic analyzer was used for bacterial identification and drug sensitivity analysis.The results were determined according to the 2016 American Clinical and Laboratory Standards Institute(CLSI).Results:From 2017 to 2021,a total of 40882 cases were submitted for examination,and 9679 positive strains were isolated.According to the type of source and proportion of the submitted specimens,the top five were sputum,blood,urine,pus,wound infection secretions and cerebrospinal fluid.Among them,there were 1551 strains of PA strains.The submitted specimens were from sputum,urine,pus,wound infection secretions and blood according to the type of source and the top 4 were sputum,urine,pus,wound infection secretions and blood.The distribution of departments was mainly based on intensive care medicine and respiratory medicine,and other departments such as hematology and oncology,urology,neurosurgery,thoracic surgery,and other departments were less isolated.The number of MDR-PA strains in 2017-2021 was 6,9,9,9,9 and 11,respectively.The total resistance rate of ceftazidime was more than 20%,the total resistance rates of other antibiotics including amikacin,imipenem,levofloxacin,cefoperazone sulbactam,ciprofloxacin,cefepime,meropenem and piperacillin/tazobactam were less than 20%.Conclusion:Pseudomonas aeruginosa isolated from our hospital is mainly sputum specimens,distributed in the department of critical care medicine and respiratory medicine.At the same time,the drug resistance of Pseudomonas aeruginosa is lower than that of domestic related literat
