摘要
长期护理保险制度作为我国社保“第六险”的制度建设之路自2016年在15个城市试点开始,并随着试点城市的不断增多而步入发展快车道。基于政策生命周期视角,总结和分析德国、日本、韩国、美国以及国内试点城市关于长期护理保险制度的产生背景、制度框架、运行机制、制度评估和政策调整等一系列政策发展过程中的经验和做法,提出我国未来要构建起以独立的长期护理保险制度为基础、商业类保险为补充的长期照护保障体系,就必须建立全国统一、独立的长期护理保险制度,提高制度覆盖的广度和保障水平;优化基础性筹资机制,确保制度的可持续性发展;鼓励发展“社会保险+商业保险”双模式,建立健全多层次长期护理保障体系;完善长期护理服务体系建设,提升照护服务水平和质量。
As the road to the system construction of“the sixth insurance”of China social security,the longterm care insurance system began in 15 pilot cities in 2016 and has entered a fast lane of development with pilot cities continuously increasing.Based on the perspective of policy lifecycle,this study summarizes and analyzes a series of the experience and practice of Germany,Japan,South Korea,the United States and the domestic pilot cities in their policy development process about long-term care insurance system,including its formulation background,system framework,operational mechanism,system evaluation and policy adjustment,and points out that in order to establish a long-term care insurance system based on an independent long-term care insurance system and supplemented by commercial insurance in the future in China,it is necessary to establish a national unified and independent long-term care insurance system and improve the breadth of system coverage and the security level,optimize the basic financing mechanism and ensure the sustainable development of the system,encourage the development of a dual model of“social insurance+commercial insurance”and establish and improve a multi-level long-term care security system,and perfect the construction of long-term care service system and enhance the level and quality of care services.
