摘要
目的 观察腹腔镜胆囊切除术(LC)对慢性胆囊炎伴胆囊结石(CCAC)患者肝功能指标及氧化应激反应的影响。方法 将2019年1月—2021年12月期间就诊于奉新县人民医院的60例CCAC患者按手术方式的不同分为2组,每组30例。对照组实施小切口胆囊切除术,观察组实施LC,观察至患者出院。比较2组临床手术相关指标、肝功能指标、氧化应激反应及并发症情况。结果 相比于对照组,观察组手术时间、术后首次排气时间及住院时间较短,术中出血量较少(P<0.05);术后3 d,2组丙氨酸转移酶(ALT)、天冬氨酸转氨酶(AST)、谷氨酰转肽酶(GGT)、总胆红素(TBIL)均高于术前,且观察组低于对照组(P<0.05);术后1 d,对照组超氧化物岐化酶(SOD)低于术前,丙二醛(MDA)高于术前,且观察组SOD高于对照组,MDA低于对照组(P<0.05);与对照组比较,观察组并发症发生率低(P<0.05)。结论 LC可减少CCAC患者术中出血量,缩短手术时间,减轻手术造成肝功能损伤及氧化应激反应,降低术后并发症发生率,利于术后恢复。
Objective To observe the effect of laparoscopic cholecystectomy(LC)on liver function indexes and oxidative stress in patients with chronic cholecystitis complicated with cholecystolithiasis(CCAC).Methods Sixty patients with CCAC who were treated in our hospital from January 2019 to December 2021 were randomly divided into two groups according to different surgical methods,30 cases in each group.The control group underwent small incision cholecystectomy,and the observation group underwent LC until the patients were discharged.The clinical operation-related indicators,liver function indicators,oxidative stress response and complications were compared between the two groups.Results Compared with the control group,the observation group had shorter operation time,postoperative first exhaust time and hospital stay,and lower intraoperative blood loss(P<0.05).It was higher than before operation,and the observation group was lower than the control group(P<0.05).1 day after operation,the SOD of the control group was lower than that of the control group,and the MDA of the control group was higher than that of the control group(P<0.05).Compared with the control group,the incidence of complications was lower in the observation group(P<0.05).Conclusion LC can reduce the intraoperative blood loss,shorten the operation time,reduce the liver function damage and oxidative stress caused by the operation,reduce the incidence of postoperative complications,and facilitate postoperative recovery in CCAC patients.
