摘要
目的:观察地屈孕酮片联合宫腔镜下息肉切除术治疗子宫内膜息肉患者的效果。方法:选取2020年1月至2022年12月该院收治的166例子宫内膜息肉患者进行前瞻性研究,按照随机数字表法将其分为对照组和研究组各83例。对照组采用宫腔镜下息肉切除术治疗,研究组在对照组基础上联合地屈孕酮片治疗,比较两组临床疗效、子宫内膜息肉复发率、子宫内膜参数指标[子宫内膜厚度、阻力指数(RI)、搏动指数(PI)]水平、血管内皮生长因子(VEGF)水平及临床症状改善指标水平。结果:研究组治疗总有效率为95.18%,高于对照组的85.54%,研究组子宫内膜息肉复发率为7.23%,低于对照组的18.07%,差异均有统计学意义(P<0.05);治疗后,两组子宫内膜厚度、RI、PI及VEGF水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);研究组阴道异常出血止血时间、腰腹疼痛缓解时间均短于对照组,月经总量少于对照组,差异有统计学意义(P<0.05)。结论:地屈孕酮片联合宫腔镜下子宫内膜息肉切除术治疗子宫内膜息肉患者,可提高临床疗效,减小子宫内膜厚度,改善子宫内膜血液循环和临床症状,降低VEGF水平和复发率,效果优于单纯宫腔镜下子宫内膜息肉切除术治疗。
Objective:To observe effects of Dydrogesterone tablets combined with hysteroscopic endometrial polypectomy in treatment of patients with endometrial polyps.Methods:A prospective study was conducted on 166 patients with endometrial polyps admitted to the hospital from January 2020 to December 2022.According to the random number table method,they were divided into control group and study group,83 cases in each group.The control group was treated with hysteroscopic endometrial polypectomy,while the study group was treated with Dydrogesterone tablets on the basis of that of the control group.The clinical efficacy,the recurrence rate of endometrial polyps,the endometrial parameter indexes levels[endometrial thickness,resistance index(RI),pulsatility index(PI)],the vascular endothelial growth factor(VEGF)level,and the clinical symptom improvement index levels were compared between the two groups.Results:The total effective rate of treatment in the study group was 95.18%,which was higher than that in the control group(85.54%);the recurrence rate of endometrial polyps in the study group was 7.23%,which was lower than that in the control group(18.07%);and the differences were statistically significant(P<0.05).After the treatment,the endometrial thickness,RI,PI and VEGF levels in the two groups were lower than those before the treatment,those in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the hemostasis time of abnormal vaginal bleeding and the relief time of lumbar and abdominal pain in the study group were shorter than those in the control group,the total amount of menstruation was less than that in the control group,and the differences were statistically significant(P<0.05).Conclusions:Dydrogesterone tablets combined with hysteroscopic endometrial polypectomy for the patients with endometrial polyps can improve clinical efficacy,reduce the endometrial thickness,improve the endometrial blood circulation and the clinical symptoms,and reduce
