放射性核素显像在心肌淀粉样变性诊疗中的研究进展

摘要 因前体蛋白异常折叠沉积于器官细胞间质引起的疾病被称为淀粉样变性,当病变累及心肌细胞时,会引发心肌淀粉样变性(CA),导致心脏结构受损,使患者心功能发生严重障碍。CA主要包括轻链型淀粉样变性和转甲状腺素蛋白淀粉样变性,不同类型CA的临床治疗存在差异,运用无创手段对CA进行早期诊断、分型,并采取积极的治疗措施,是改善患者预后的关键。放射性核素显像可对CA进行无创诊断,在CA分型、患者危险度分层、疗效评估方面具有良好的应用价值。目前,放射性核素显像在CA中的研究深度不够,相关研究结论有待进一步验证。而利用放射性核素显像筛选优势患者群体进行药物治疗、疗效监测及显像分子探针的开发等,在未来可能成为新的研究热点。 The disease caused by abnormal folding and deposition of precursor proteins in the interstitium of organ cells is called amyloidosis.When the lesions affect myocardial cells,cardic amyloidosis(CA)occurs,resulting in damage to heart structure and serious cardiac dysfunction.CA mainly includes light chain amyloidosis and transthyretin amyloidosis.The clinical treatments of different types of CA are different.Early diagnosis and classification of CA by non-invasive means,and active treatment measures are important to the prognosis improvement.Radionuclide imaging can be used for noninvasive diagnosis,classification,risk stratification,and efficacy evaluation of CA.Currently,the application of radionuclide imaging in the diagnosis and treatment of CA has not been studied deeply,relevant research conclusions need further verification.The application of radionuclide imaging in screening dominant patients for drug therapy,monitoring efficacy and developing imaging molecular probes may become the research hotspots in the future.
