摘要
目的了解老年糖尿病患者糖尿病痛苦现状并分析其影响因素。方法以糖尿病痛苦量表(DDS)、社会支持评定量表(SSRS)、医学应对方式问卷(MCMQ)和自制一般资料调查表作为研究工具,对2022年3~9月在南宁市某三甲医院住院治疗的老年糖尿病患者进行问卷调查。结果321例老年糖尿病患者中,122例(38.01%)患者有糖尿病痛苦。不同年龄、学历、职业、个人月均收入、运动情况,不同血糖监测情况、患病时长、合并症情况、并发症情况、治疗方式,以及不同社会支持水平和应对方式的老年糖尿病患者的糖尿病痛苦情况差异均有统计学意义(均P<0.05)。二分类logistic回归分析结果表明:高龄、患病时长长、社会支持水平高是老年糖尿病患者发生糖尿病痛苦的保护因素,不规律监测血糖、采用口服药物+胰岛素治疗、采用回避应对方式是老年糖尿病患者痛苦的危险因素(均P<0.05)。结论老年糖尿病患者糖尿病痛苦发生率较高,高龄、患病时长长、社会支持水平高均是老年糖尿病患者发生糖尿病痛苦的保护因素,不规律监测血糖、采用口服药物+胰岛素治疗、采用回避应对方式均是其危险因素。
Objective To investigate the status quo of diabetes distress in elderly diabetic patients and to analyze its influencing factors.Methods Diabetes Distress Scale(DDS),Social Support Rate Scale(SSRS),Medical Coping Modes Questionnaire(MCMQ),and self-made general information questionnaire were used as research tools to survey elderly diabetic patients who were hospitalized in a Grade-A Tertiary Hospital in Nanning from March to September 2022.Results Among the 321 elderly diabetic patients,122(38.01%)patients had diabetes distress.There were statistically significant differences in diabetes distress of elderly diabetic patients with different ages,educational backgrounds,occupations,average personal monthly incomes,exercise,blood glucose monitoring,duration of illness,comorbidities,complications,treatments,social support levels,and coping modes(all P<0.05).The results of binary logistic regression analysis showed that old age,a long duration of illness,and a high social support level were protective factors for diabetes distress in elderly diabetic patients,and irregular blood glucose monitoring,being treated with oral drugs and insulin,and avoidance coping mode were risk factors for diabetes distress in elderly diabetic patients(all P<0.05).Conclusions The incidence of diabetes distress in elderly diabetic patients is high,and old age,a long duration of illness,and a high social support level are protective factors for diabetes distress,and irregular blood glucose monitoring,being treated with oral drugs and insulin,and avoidance coping mode are risk factors in elderly diabetic patients.
