摘要
目的:探究腹腔镜手术联合亮丙瑞林、地屈孕酮、阿司匹林在子宫内膜异位症不孕患者中的应用效果。方法:选取2018年8月—2022年8月华侨大学附属泉州德诚医院收治的80例子宫内膜异位症不孕患者为研究对象,根据随机数表法分为对照组和观察组,各40例。对照组进行腹腔镜手术治疗,观察组进行腹腔镜手术联合亮丙瑞林、地屈孕酮、阿司匹林治疗。比较两组的治疗总有效率、治疗前后的子宫内膜异位症生育指数(EFI)评分及子宫内膜容受性指标[子宫内膜厚度、子宫内膜容积、子宫内膜形态及子宫内膜下螺旋动脉搏动指数(PI)及阻力指数(RI)]。结果:观察组治疗总有效率显著高于对照组,差异有统计学意义(P<0.05)。治疗3个月及6个月后,观察组EFI评分显著优于对照组,差异有统计学意义(P<0.05)。治疗3个月及6个月后,观察组子宫内膜厚度及子宫内膜容积显著高于对照组,差异有统计学意义(P<0.05)。治疗3个月及6个月后,观察组的子宫内膜形态显著优于对照组,差异有统计学意义(P<0.05)。治疗3个月及6个月后,观察组的子宫内膜下螺旋动脉PI及RI显著低于对照组,差异有统计学意义(P<0.05)。结论:腹腔镜手术联合亮丙瑞林、地屈孕酮、阿司匹林在子宫内膜异位症不孕者中的应用效果较好,可显著改善患者的子宫内膜容受性及EFI评分。
Objective:To investigate the application effect of laparoscopic surgery combined with Leuprorelin,Dydrogesterone and Aspirin in the patients with endometriosis-associated infertility.Method:A total of 80 patients with endometriosis-associated infertility in Quanzhou Decheng Hospital Affiliated to Overseas Chinese University from August 2018 to August 2022 were selected as the study objects,and they were divided into control group and observation group by the random number table method,40 cases in each group.The control group were treated with laparoscopic surgery,the observation group were treated with laparoscopic surgery combined with Leuprorelin,Dydrogesterone and Aspirin.Then the total effective rates,endometriosis fertility index(EFI)score and endometrial receptivity indexes[endometrial thickness,volume,morphology,pulsatility index(PI)and resistance index(RI)of endometrial spiral arteries]before and after the treatment of two groups were compared.Result:The total effective rate of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).After 3 and 6 months of treatment,EFI scores in the observation group were significantly better than those in the control group,and the differences were statistically significant(P<0.05).After 3 and 6 months of treatment,endometrial thickness and endometrial volume in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).After 3 and 6 months of treatment,the endometrial morphology of the observation group were significantly better than those of the control group,and the differences were statistically significant(P<0.05).After 3 and 6 months of treatment,the PI and RI of the lower endometrial spiral artery in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:The application effect of laparoscopic surgery combined with L
