摘要
目的探讨超声辅助下直入穿刺法与侧入穿刺法蛛网膜下腔阻滞术后并发症发生情况。方法选取2022年3月—9月于抚州市东乡区人民医院超声辅助下行椎管内麻醉的100例患者,按随机数字表分为两组,各50例。对照组予以超声辅助下直入穿刺法蛛网膜下腔阻滞,观察组予以超声辅助下侧入穿刺法蛛网膜下腔阻滞。比较两组生命体征变化及术后并发症情况。结果两组麻醉前(T0)、麻醉30 min(T1)、缝合切口时(T2)及术毕30 min(T3)时的心率(HR)、平均动脉压(MAP)相比,差异无统计学意义(P>0.05);观察组术后头痛、神经症状发生率分别为4.0%、8.0%,低于对照组的16.0%、24.0%,差异有统计学意义(P<0.05)。结论超声辅助下直入穿刺法与侧入穿刺法蛛网膜下腔阻滞均能取得理想麻醉效果,可稳定术中生命体征波动,但侧入穿刺法术后并发症风险低,值得临床广泛应用。
Objective To investigate the postoperative complications of ultrasound-assisted direct puncture and lateral puncture in subarachnoid block.Methods A total of 100 patients with ultrasound-assisted spinal anesthesia in Dongxiang District People's Hospital of Fuzhou from March to September were selected and divided into two groups according to random number table,with 50 patients in each group.Control group were given subarachnoid block by ultrasound-assisted direct puncture,and observation group were given subarachnoid block by ultrasound-assisted lateral puncture.And then,the changes of vital signs and postoperative complications were compared between the two groups.Results There were no statistically significant differences in heart rate(HR)and mean arterial pressure(MAP)between the two groups before anesthesia(T0),30 min of anesthesia(T1),time of suturing incision(T2)and 30 minutes after surgery(T3)(P>0.05).The incidence of postoperative headache and neurological symptoms in the observation group were 4.0%and 8.0%,respectively,which were lower than those in the control group(16.0%and 24.0%),and the difference was statistically significant(P<0.05).Conclusion Both ultrasound-assisted direct puncture and lateral puncture in subarachnoid block can achieve ideal anesthesia effects and stabilize the fluctuation of vital signs during surgery.However,the risk of complications after lateral puncture is low,which is worthy of clinical application.
