成人髂嵴连线与脊柱相交处定位L3,4棘突间隙或L4棘突的准确性 被引量:6

Accuracy of intersection of intercristal line and the spine in determining L3,4 interspace or L4 spinous process in adults

摘要 目的评价成人髂嵴连线与脊柱相交处定位L3,4棘突间隙或L4棘突的准确性。方法择期行脊柱手术患者184例,年龄24~82岁,体重50~80妇,取屈髋屈膝左侧卧位,标记髂嵴连线与脊柱相交处,在C型臂机下确认标记处所在脊柱的位置。C型臂机下标记处位于L3,4棘突间隙或L4棘突为定位准确,记录定位不准确的发生情况。结果髂嵴连线位于L3,4棘突间隙至L5棘突水平,男性较女性向头侧移位(P〈0.05),定位不准确率18%。结论成人髂嵴连线与脊柱相交处作为确定L3,4棘突间隙或L4棘突的定位标志,不能确保所选位置的准确性,需要慎重。 Objective To determine how often the line joining both iliac crests intersects the spine at L3,4 interspace or L4 spinous process. Methods One hundred and eighty-four patients aged 24-82 yr weighing 50-80 kg scheduled for spine surgery were placed in left lateral position with their hips and knees flexed. A line joining both iliac crests was drown. A metal marker was placed at the point where the intercristal line intersects the spine and detected by mobile X-ray system. The intersection was considered accurately located when the marker lay on L3,4 interspace or L~ spinous process. The rate of inaccuracy was calculated. Results The intercristal line lay between Lz,3 interspace and Ls spinous process and lay more cephalad in male than in female. The inaccuracy rate was 18%. Conclusion The location of intersection of intercristal line and spine should be checked cautiously before lumbar puncture.
