摘要
目的 探究糖尿病足感染(DFI)患者下肢血管病变(PAD)与免疫功能及营养状态的关系。方法 回顾性分析2020年1月至2022年1月西安市第九医院收治的104例DFI患者的临床资料。根据患者是否发生PAD分为病变组(n=66)和非病变组(n=38),比较两组患者的免疫功能[免疫球蛋白A (IgA)、免疫球蛋白G (IgG)、免疫球蛋白M (IgM)]及营养状态相关指标,采用多因素Logistic回归分析DFI患者PAD的影响因素;根据病变组患者的PAD程度不同,将其分为A、B、C、D四个等级,比较其免疫功能与营养状态指标。结果 病变组患者的血红蛋白(HB)、甘油三酯(TG)、总胆固醇(TC)、IgA、IgG以及IgM水平明显低于非病变组,血清肌酐(Cr)水平明显高于非病变组,差异均有统计学意义(P<0.05);经多因素Logistic回归分析结果显示,HB、Cr、IgA、IgG及IgM是DFI患者PAD的独立影响因素(P<0.05);不同PAD等级DFI患者的IgA、IgG、IgM、HB及Cr水平比较,差异均有统计学意义(P<0.05)。结论 DFI发生PAD患者免疫功能、营养状态明显下降,均为PAD的独立影响因素,且与PAD严重程度密切相关,临床上应给予重点关注。
Objective To investigate the relationship between peripheral arterial disease(PAD)and immune function and nutritional status in patients with diabetic foot infection(DFI).Methods This study retrospectively re-viewed 104 patients with DFI admitted to the Ninth Hospital of Xi'an from January 2020 to January 2022.According to the presence or absence of PAD,the patients were divided into PAD group(n=66)and non-PAD group(n=38).Immune function[immunoglobulin A(IgA),immunoglobulin G(IgG),and immunoglobulin M(IgM)]and nutritional indicators were compared between the two groups.Multivariate logistic regression analysis was performed to screen the influenc-ing factors of PAD in patients with DFI.Patients in the PAD group were divided into 4 grades(A,B,C,D)according to the severity of PAD.Immune function and nutritional indicators in patients with different grades of PAD were compara-tively analyzed.Results The levels of hemoglobin(HB),triglyceride(TG),total cholesterol(TC),IgA,IgG,and IgM in the PAD group were significantly lower than those in the non-PAD group.Serum creatinine(Cr)level was significant-ly higher than that in the non-PAD group(P<0.05).Multivariate logistic regression analysis showed that HB,Cr,IgA,IgG,and IgM were independent influencing factors of PAD in patients with DFI(P<0.05).There were statistically signifi-cant differences in the levels of IgA,IgG,IgM,HB,and Cr among different grades of PAD(P<0.05).Conclusion The im-mune function and nutritional status in DFI patients with PAD are significantly decreased.They are independent influencing factors of PAD and are closely related to the severity of PAD,which should be paid close attention to.
