摘要
目的全面调查和分析全国细菌耐药监测网点医院临床微生物实验室现状。方法基于全国细菌耐药监测网(CARSS)中参与调查的4513家医院提供的微生物实验室建设及耐药监测能力数据信息,选取2021年新入网医院(3096家)和2021年前已加入CARSS的原国网医院(1417家)中同样规模床位数的1051家和513家,对其微生物实验室检测空间、仪器设备配置及检测方法、微生物检验、培养及药敏试验开展和质量控制等情况进行数据统计和分析。结果2021年CARSS新入网同等规模床位的二、三级医院微生物实验室面积分别为(50.4±40.6)和(91.1±60.3)m^(2),原国网医院分别为(56.9±44.8)和(122.7±87.5)m^(2);新入网医院每百张床位人员配备数分别为3(3,5)人和4(3,6)人,原国网医院分别为3(2,4)人和5(4,7)人。仪器设备配置方面,原国网医院的质谱仪、CO_(2)培养箱、低温冰箱的配备率分别为33.1%(170/513)、89.1%(457/513)和75.4%(387/513),高于新入网医院[分别为4.8%(50/1051)、65.4%(687/1051)和51.4%(540/1051),P均<0.001];在室间质评参评率和室内质控开展率上,原国网医院分别为78.4%(402/513)和93.8%(481/513),均高于新入网医院的19.2%(202/1051)和61.4%(645/1051)(P<0.001)。结论在基础设施、人员配备、检测能力、质量控制等环节均显示已参与CARSS监测工作的原国网医院各方面均优于新入网医院,参加监测工作可在同一标准的推动下,不断改进实验室能力,促进微生物检验学科的发展。
Objective To investigate the current situation of the clinical microbiology laboratories in membership hospitals of the China Antimicrobial Resistance Surveillance System(CARSS).Methods Data of microbial laboratory construction and drug resistance monitoring capacity were screened based on report from the 4513 membership hospitals.Data of 1051 new membership hospitals,referring hospitals joined the CARSS after 2021 and 513 former membership hospitals,referring hospitals joined CARSS before 2021 were analyzed.Analyzed data included infrastructure,microbiological testing,culture,drug sensitivity testing,and quality control.Results Of the newly admitted secondary and tertiary hospitals′microbial laboratories,the average space was(50.4±40.6)m^(2) and(91.1±60.3)m^(2),respectively,and those of the former membership hospitals were(56.9±44.8)and(122.7±87.5)m^(2),respectively;Of the average staff number per 100 beds,the median of the newly admitted secondary and tertiary hospitals′microbial laboratories were 3(3,5)and 4(3,6)compared with 3(2,4)and 5(4,7)of the former ones.In terms of infrastructure,the equipped rates of mass spectrometers(170,33.1%),CO_(2)incubators(457,89.1%),and low-temperature refrigerators(387,75.4%)of the former membership hospitals were higher than those of newly admitted hospitals(P<0.001);regarding external quality assessment and internal quality control,both of the assessment participation rate and the quality control rate of the former membership hospitals[78.4%(402/513),93.8%(481/513)]were significantly higher than newly admitted hospitals[19.2%(202/1051),61.4%(645/1051),P<0.001].Conclusions The former members of CARSS are superior to the new members in terms of infrastructure,staffing,detection capabilities,and quality control.Participating in the monitoring work of CARSS can continuously improve the laboratory ability and promote the development of microbiological examination discipline under standard principles.
