摘要
目的:比较甲状腺癌患者采用甲状腺全切除术与近全切除术的治疗效果。方法:选取南京市高淳中医院2021年1月—2022年9月收治的甲状腺癌患者62例作为研究对象,根据手术方式分为研究组与对照组,各31例。对照组实施甲状腺全切除术,研究组实施甲状腺近全切除术。比较两组临床疗效、临床指标、并发症发生情况、复发情况、生化指标及生活质量。结果:研究组治疗有效率高于对照组,差异有统计学意义(P=0.023)。研究组手术时间、镇痛时间、住院时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。研究组并发症总发生率低于对照组,复发率高于对照组,差异有统计学意义(P<0.05)。术后1 d,研究组甲状旁腺激素、血清钙离子水平高于对照组,差异有统计学意义(P<0.001)。术后1个月,研究组躯体、心理及认知评分高于对照组,差异有统计学意义(P<0.05)。结论:相较于甲状腺全切除术,甲状腺癌患者进行甲状腺近全切除术的临床疗效更佳,可改善临床指标,降低并发症发生率,减小对甲状旁腺激素、血清钙离子水平的影响,提高生活质量,但复发率较高。
Objective:To compare the therapeutic effect of total thyroidectomy and near-total thyroidectomy in patients with thyroid cancer.Methods:A total of 62 patients with thyroid cancer admitted to Nanjing Gaochun Hospital of Traditional Chinese Medicine from January 2021 to September 2022 were selected as the study subjects.They were divided into study group and control group according to the operation approach,with 31 cases in each group.The control group implemented total thyroidectomy,and the study group implemented near-total thyroidectomy.The clinical efficacy,clinical indexes,complications,recurrence,biochemical indexes and quality of life were compared between the two groups.Results:The treatment response rate in the study group was higher than that in the control group,and the difference was statistically significant(P=0.023).The operation time,analgesia time and hospital stay in the study group were shorter than those in the control group,and the intraoperative blood loss in the study group was less than that in the control group,and the difference was statistically significant(P<0.05).The total incidence of complications in the study group was lower than that in the control group,and the recurrence rate in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).On 1 d after surgery,the levels of parathyroid hormone and serum calcium ions in the study group were higher than those in the control group,and the difference was statistically significant(P<0.001).At 1 month after surgery,the physical,psychological and cognitive scores in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Compared with total thyroidectomy,near-total thyroidectomy in patients with thyroid cancer has a better clinical effect,which can improve clinical indicators,reduce the incidence of complications,reduce the impact on parathyroid hormone and serum calcium ion levels,and improve the quality of life,bu
