摘要
目的探讨参芪麦味地黄汤联合穴位敷贴治疗2型糖尿病(T2DM)的效果。方法选取2020年5月至2021年6月贵溪市中医院收治的63例T2DM患者作为研究对象,按照抽签法分为对照组(30例)与观察组(33例)。对照组给予糖尿病饮食、运动、二甲双胍等常规治疗,观察组在对照组基础上加服参芪麦味地黄汤联合穴位敷贴治疗。比较两组患者治疗前后血清的单核细胞趋化蛋白-1(MCP-1)、白介素-6(IL-6)水平、空腹血糖(FPG)、餐后2 h血糖(2 hPG)、胰岛β细胞功能[胰岛素抵抗指数(HOMA-IR)、胰岛素分泌指数(HOMA-β)]、糖化血红蛋白A1c(HbA1c)及血流动力学指标变化情况。结果观察组患者治疗后的FPG、2 hPG、HbA1c低于对照组,差异有统计学意义(P<0.05);观察组患者治疗后的HOMA-β高于对照组,血清MCP-1、IL-6水平、HOMA-IR指数低于对照组,差异有统计学意义(P<0.05);观察组患者治疗后的心输出量(CO)、每搏量(SV)、心排指数(CI)水平均高于对照组,差异均有统计学意义(P<0.05)。结论参芪麦味地黄汤联合穴位敷贴能够有效降低T2DM患者血糖及HbA1c水平,改善胰岛β细胞功能,疗效显著,其机制可能与抑制MCP-1、IL-6炎症因子释放,调节血液循环,改善血瘀症状有关。
Objective To investigate the effect of Shenqi Maiwei Dihuang Decoction combined with acupoint application on type 2 diabetes mellitus(T2DM).Methods A total of 63 T2DM patients admitted to Guixi Hospital of Traditional Chinese Medicine from May 2020 to June 2021 were selected as study subjects.They were divided into control group(30 cases)and observation group(33 cases)by drawing lots.The control group was given conventional treatment of diabetic diet,exercise and metformin.Observation group was treated with Shenqi Maiwei Dihuang Decoction combined with acupoint application based on control group.Serum levels of monocyte chemoattractant protein-1(MCP-1),interleukin-6(IL-6),fasting blood glucose(FPG),2 h blood glucose(2 hPG),isletβcell function(insulin resistance index[HOMA-IR],insulin secretion index[HOMA-β])and glycosylated hemoglobin A1c(HbA1c)were compared between the two groups before and after treatmentlc)and changes of hemodynamic indexes.Results After treatment,FPG,2 hPG and HbAlc in observation group were lower than those in control group,and the differences were statistically significant(P<0.05).After treatment,HOMA-βof observation group was higher than control group,serum MCP-1,IL-6 levels and HOMA-IR index were lower than control group,the differences were statistically significant(P<0.05).After treatment,the cardiac output(CO),stroke volume(SV)and cardiac output index(CI)in observation group were higher than those in control group,with statistical significance(P<0.05).Conclusion Shenqi Maiwei Dihuang Decoction combined with acupoint application can effectively reduce blood glucose and HbA1c levels in T2DM patients,improve the function of pancreaticβcells,and the effect is significant.The mechanism may be related to inhibiting the release of MCP-1 and IL-6 inflammatory factors,regulating blood circulation,and improving blood stasis symptoms.
