摘要
目的对某院静脉药物集中配置中心(PIVAS)不合理医嘱进行帕累托图分析,为更好地规范医嘱的合理应用提供参考。方法通过前置审方系统,对2022年1—6月医院PIVAS用药医嘱进行统计,运用帕累托图进行分析。结果共审查了60783组医嘱,其中3467组不合理,占5.7%。科室分布中,A类因素为骨科、普外科、感染科、肿瘤科、放疗科、血液科(共2606组,占75.2%),B类因素为呼吸科(314组,占9.1%);类型分布中,A类因素为给药频次不当、溶媒选择不当、给药途经不当(共2670组,占77.0%),B类因素为溶媒量多(431组,占12.4%)。结论利用帕累托图得出PIVAS不合理医嘱中的主、次要因素,审方药师前置审方可针对性干预,促进临床合理用药。
Objective The Pareto diagram analysis of unreasonable medical orders in PIVAS of a hospital was carried out to provide reference for standardizing the rational application of medical orders.Methods Through the pre-prescription review system,the PIVAS medication orders of a hospital from January to June 2022 were statistically analyzed by Pareto diagram.Results A total of 60783 medical orders were reviewed,of which 3467(accounting for 5.7%)were unreasonable.In the distribution of departments,category A factors were orthopedics,general surgery,infectious diseases,oncology,radiotherapy and hematology(a total of 2606 groups,accounting for 75.2%),and category B factors were respiratory(314 groups,accounting for 9.1%).Among the type distribution,class A factors were improper administration frequency,improper solvent selection and improper administration route(2670 groups,accounting for 77.0%),and class B factors were high solvent quantity(431 groups,accounting for 12.4%).Conclusion The primary and secondary factors of unreasonable PIVAS orders are obtain by Pareto diagram,and only target intervention can be carry out by prescription examiners in advance to promote rational clinical drug use.
