提高肾内科住院医师规范化培训教学查房质量实践 被引量:3

Practical Exploration of Improving the Standardized Residency Training Quality of Teaching Rounds in the Department of Nephrology

摘要 目的探讨肾内科教学查房改革实践对培养规范化培训住院医师(规培医师)的应用效果,为提高教学查房质量提供依据。方法将2019年11月至2021年6月轮转肾内科的学员设为对照组14人和实验组18人。对照组采用传统教学查房模式。实验组在以下三方面进行教学查房改革实践:建立基于标准操作规范的肾内科教学查房制度;教学查房中进行分层教学以及教学查房模式的多样化尝试,后者包括日常工作查房中的教学查房以及由规培医师作为主体来完成的病例报告。采用360度评估系统和出科临床实践能力考核成绩评估教学效果。采用SPSS17.0软件进行统计学分析。结果在提高教学查房质量实践效果评估中,指标“是否有较强的教学能力”、“教学查房和专业讲座准备充分,深入浅出,条理清楚”的评分以及总平均分由对照组(4.66±0.19)分、(4.57±0.21)分和(4.71±0.07)分提升至实验组(4.83±0.21)分、(4.81±0.18)分和(4.84±0.10)分(P<0.05),规培医师在临床实践能力考核中的平均成绩由对照组(91.0±2.9)分提升至实验组(93.6±2.5)分(P<0.05)。结论教学查房改革实践有助于提高教学查房质量,提升规培医师的满意度和临床实践能力。 Objectives To explore the application effect of the reform practice of teaching rounds on the training residents in the department of nephrology,so as to provide basis for improving the quality of teaching rounds.Methods The training residents from November 2019 to June 2021 were divided into two groups:the control group(n=14)and the experimental group(n=18).The traditional mode of teaching rounds was adopted in the control group.The experimental group carried out teaching rounds in the following three aspects:establishing the teaching rounds system based on the standard operation procedure(SOP)in the department of nephrology;applicating hierarchical teaching method and diversifying the mode of teaching rounds.The latter included the teaching rounds in the daily morning rounds and the case reported completed by the training residents as the main body.The teaching effect was evaluated by the 360-degree evaluation system and clinical practice ability examination.SPSS17.0 software was used for statistical analysis.Results In the effect evaluation of improving the quality of teaching rounds,the score of“better teaching ability”,“teaching rounds and professional lectures were well prepared,easy to understand and well organized”,and the total average score increased from(4.66±0.19),(4.57±0.21)and(4.71±0.07)in the control group to(4.83±0.21),(4.81±0.18)and(4.84±0.10)in the experimental group(P<0.05).The average score of residents'clinical practice ability examination increased from(91.0±2.9)to(93.6±2.5)(P<0.05).Conclusions The reform practice of teaching rounds could improve the quality of teaching rounds and enhance the satisfaction and clinical practice ability of the training residents.
