酰胺质子转移加权成像特征及联合常规磁共振扫描对低级别脑膜瘤的诊断研究

摘要 目的探究不同亚型低级别脑膜瘤酰胺质子转移加权成像(APT)成像特征及联合常规磁共振扫描的诊断研究。方法2018年2月至2023年2月海南省人民医院收治的WHO I级脑膜瘤患者80例,病理检查确诊WHO I级脑膜瘤,行磁共振(MRI)常规平扫和APT扫描。比较不同亚型低级别脑膜瘤患者的T1WI信号强度、T2WI信号强度、瘤旁脑水肿(PTBE)信号强度、瘤核心区APT强度最大值(APTmax)、最小值(APTmin)和均值(APTmean)。分析MRI APT成像特征参数及常规磁共振扫描参数对不同亚型低级别脑膜瘤的诊断价值。结果不同病理亚型的低级别脑膜瘤T1WI信号、T2WI信号表现差异有统计学意义(P<0.05);PTBE信号表现无统计学意义(P>0.05)。不同病理亚型的APTmax、APTmin及APTmean比较,差异有统计学意义(P<0.05)。MRI APT成像特征参数联合T1WI信号及T2WI信号诊断脑膜瘤亚型的曲线下面积(AUC)分别为0.938(95%CI:0.875~0.998)、0.880(95%CI:0.768~0.992)、0.917(95%CI:0.791~1.000)、1.000(95%CI:1.000~1.000)。结论MRI APT成像特征参数及常规磁共振扫描参数在诊断低级别脑膜瘤的不同亚型中的效能良好。 Objective To explore the imaging characteristics of amide proton transfer weighted imaging(APT)in different subtypes of low-level meningiomas and the diagnostic study in combination with conventional magnetic resonance imaging.Methods Between February 2018 and February 2023,there were 80 patients with WHO grade I meningioma admitted to Hainan Central Hospital and confirmed WHO grade I meningioma after pathological examination,and routine magnetic resonance imaging(MRI)plain scan and APT scan were performed.T1WI signal intensity,T2WI signal intensity,paratumoral brain edema(PTBE)signal intensity,APTmax,APTmin,and APTmean of APT intensity in different subtypes of low-grade meningioma patients were compared.The diagnostic value of MRI APT imaging characteristic parameters and conventional magnetic resonance scanning parameters for different subtypes of low-level meningiomas was analyzed.Results There was a statistically significant difference in T1WI and T2WI signal expression among low-grade meningiomas of different pathological subtypes(P<0.05).The PTBE signal expression was not statistically significant(P>0.05).The differences in APTmax,APTmin,and APTmean among different pathological subtypes were statistically significant(P<0.05).The area under the curve(AUC)of MRI APT imaging feature parameters combined with T1WI and T2WI signals for diagnosing meningioma subtypes were 0.938(95%CI:0.875-0.998),0.880(95%CI:0.768-0.992),0.917(95%CI:0.791-1.000),and 1.000(95CI%:1.000-1.000),respectively.Conclusions The effectiveness of MRI APT imaging feature parameters and conventional magnetic resonance scanning parameters in diagnosing different subtypes of low-level meningioma is good.
