摘要 目的:探讨分析磁共振成像液体衰减反转恢复(FLAIR)血管高信号征与急性脑梗死患者神经功能、梗死面积及血管狭窄程度相关性。方法:选择2019年1月~2021年1月来本院急诊的59例急性脑梗死患者,患者入组24h内进行磁共振检查,收集患者FLAIR血管高信号征(FHV),根据患者是否有FHV分为FHV组34例、非FHV组55例。比较两组患者美国国立卫生研究院卒中量表(NIHSS)评分、脑梗死体积以及血管狭窄率,并分析FHV组患者ASPECT评分与NIHSS评分、脑梗死体积以及血管狭窄率相关性。结果:FHV组患者NIHSS评分、脑梗死体积以及血管狭窄率均显著高于无FHV组(P<0.05)。相关性分析结果显示,FHV组患者ASPECT评分与NIHSS评分、脑梗死体积及血管狭窄率均呈显著正相关关系(P<0.05)。结论:磁共振成像FLAIR血管高信号征与患者神经功能缺损、脑梗死体积以及血管狭窄程度密切相关,可成为急性脑梗死患者疾病严重程度判定的临床指标之一。 Objective:To investigate and analyze the correlation between FLAIR vascular hyperintensity sign on magnetic resonance imaging and neurological function score,infarct size and degree of vascular stenosis in patients with acute cerebral infarction.Methods:A total of 59 patients with acute cerebral infarction who came to the emergency department of our hospital from January 2019 to January 2021 were selected.The patients underwent magnetic resonance examination within 24 hours of enrollment,and the FLAIR vascular hyperintensity sign(FHV)was collected.Divided into FHV group 34 cases,non-FHV group 55 cases.The neurological deficit score(NIHSS),cerebral infarction volume and vascular stenosis rate were compared between the two groups,and the correlation between the ASPECT score and NIHSS,cerebral infarction volume and vascular stenosis rate in the FHV group was analyzed.Results:The NIHSS score,cerebral infarction volume and vascular stenosis rate in the FHV group were significantly higher than those in the non-FHV group(P<0.05).The results of correlation analysis showed that there was a significant positive correlation between ASPECT score and NIHSS score,cerebral infarction area and vascular stenosis rate in FHV group(P<0.05).Conclusion:FLAIR is closely related to neurological impairment,infarct volume and the degree of vascular stenosis in patients with acute cerebral infarction,and can be one of the clinical indicators to determine the severity of disease in patients with acute cerebral infarction.
