血液净化在急性敌草快中毒中的应用现状和研究进展

摘要 敌草快(DQ)是目前较为常用的非选择性除草剂,因其可通过多种途径被人体吸收,使其在应用过程中易发生中毒事件,此外因无特效解毒药物,其死亡率仍较高,疗效及预后欠佳。近年来,血液净化逐渐成为治疗中的重要手段,血液透析(HD)和血液灌流(HP)等多种方法逐渐在临床应用中开展,但目前仍无足够临床证据支持。通过国内外相关文献进行检索并归纳总结表明,血液净化是敌草快中毒的有效治疗手段,其中血液灌流是最为常用的手段,但对于标准方式及时机的选择仍无定论。近年来杂合式血液净化模式在急性重症中毒患者救治中的应用优势逐渐凸显,能够持续快速地清除血液中的有毒物质达到最佳治疗效果,各种血液净化手段的联合使用是必然趋势,对于如何合理优化组合亦是研究应用的重点。 Diquat(DQ)is a relatively commonly used non-selective herbicide at present.Because it can be absorbed by the human body through various ways,it is prone to poisoning events in the application process.In addition,because there is no specific detoxification drug,its mortality rate is still high,and its curative effect and prognosis are poor.In recent years,blood purification has become an important means of treatment.Hemodialysis(HD)and hemoperfusion(HP)have ever been used in clinical applications,but there is still no enough clinical evidence to support this dialy⁃sis.Through the search and summary of relevant literature at home and abroad,it is shown that blood purification is an effective treatment of dipleulocea poisoning,among which hemoperfusion is the most commonly used means,but there is still no conclusion on the choice of standard method and timing.In recent years,the application advantage of hetero⁃zygous blood purification mode in the treatment of acute severe poisoning patients has gradually become prominent,which can continuously and rapidly remove toxic substances in the blood to achieve the best therapeutic effect.The combined use of various blood purification means is an inevitable trend,and how to optimize the combination is also the focus of research and application.
