摘要
目的 研究妊娠期糖尿病检验中空腹血糖、餐后血糖和糖化血红蛋白联合的临床诊断价值。方法 将2021年10月—2022年10月在山东省菏泽市牡丹人民医院接受治疗的妊娠期糖尿病患者60例作为观察组,另选取同时间段内在本院行孕期检查的血糖正常孕妇60例作为对照组。两组研究对象均接受空腹血糖、餐后血糖、糖化血红蛋白检测,将两组的空腹血糖、餐后血糖(餐后1 h、餐后2 h)、糖化血红蛋白检测值进行组间比较,计算糖化血红蛋白、空腹血糖和餐后血糖的诊断灵敏度、特异度和准确度。结果 观察组空腹血糖、餐后1 h血糖、餐后2 h血糖值均显著高于对照组,差异有统计学意义(P<0.05)。观察组糖化血红蛋白水平高于对照组,差异有统计学意义(P<0.05)。空腹血糖的特异度、灵敏度、准确度分别是85.00%、88.33%、86.67%;餐后血糖的特异度、灵敏度、准确度分别是70.00%、73.33%、71.67%;糖化血红蛋白的特异度、灵敏度、准确度分别是71.67%、75.00%、73.33%;联合诊断的特异度、灵敏度、准确度分别是98.33%、96.67%、97.50%。结论 在妊娠期糖尿病患者的临床检验上,空腹血糖、餐后血糖和糖化血红蛋白联合诊断有较高的诊断价值,诊断准确度、灵敏度和特异度高,可降低误诊与漏诊的风险。
Objective To study the clinical diagnostic value of fasting blood glucose,postprandial blood glucose and glycosylated hemoglobin in the detection of gestational diabetes mellitus.Methods From October 2021 to October 2022,60 patients with gestational diabetes treated in Heze Mudan People's Hospital,Shandong Province were selected as the observation group of this study.In addition,60 pregnant women with normoglycemia who underwent pregnancy examination in the hospital during the same period were selected as the control group.Both groups of subjects were tested for fasting blood glucose,postprandial blood glucose,and glycosylated hemoglobin,and the detection values of fasting blood glucose,postprandial blood glucose(1-hour postprandial and 2-hour postprandial),and glycosylated hemoglobin were compared between the two groups.The diagnostic sensitivity,specificity and accuracy of glycosylated hemoglobin,fasting blood glucose and postprandial blood glucose were calculated.Results The fasting blood glucose,1-hour postprandial blood glucose,and 2-hour postprandial blood glucose of the observation group were significantly higher than those of the control group,and the difference was statistically significant(P<0.05).The detection value of glycated hemoglobin in the observation group was greater than that in the control group,and the difference was statistically significant(P<0.05).The specificity,sensitivity,and accuracy of fasting blood glucose were 85.00%,88.33%,and 86.67% respectively.The specificity,sensitivity,and accuracy of postprandial blood glucose were 70.00%,73.33%,and 71.67%,respectively.The specificity,sensitivity,and accuracy of glycosylated hemoglobin were 71.67%,75.00%,and 73.33%,respectively.The specificity,sensitivity,and accuracy of combined diagnosis were significantly 98.33%,96.67%,and 97.50%,respectively.Conclusion In the clinical examination of patients with gestational diabetes mellitus,the combined diagnosis of fasting blood glucose,postprandial blood glucose,and glycosylated hemoglobin has a high
