摘要
目的探究miR-223在儿童咳嗽变异性哮喘(CVA)中的水平与炎症和免疫相关性及诊断价值。方法选取2020年9月至2022年11月就诊于海南现代妇女儿童医院的咳嗽变异性哮喘患儿53例为观察组,另选取门诊健康体检儿童53例为对照组。测量微小RNA(miR)-223、T辅助细胞(Th)1、Th2、Th1/Th2、白介素(IL)-2、IL-6和肿瘤坏死因子(TNF)-α水平。结果观察组mi R-223水平高于对照组,Th1、Th2和Th1/Th2水平低于对照组(P_(均)<0.05)。观察组IL-2水平低于对照组,IL-6和TNF-α水平均高于对照组(P_(均)<0.05)。血清miR-223水平与Th1、Th2、Th1/Th2、IL-2水平呈显著负相关(r=–0.675、–0.550、–0.565、–0.618;P_(均)<0.001),与IL-6、TNF-α呈显著正相关(r=0.686、0.706;P_(均)<0.001)。多因素Logistic回归表明,miR-223、IL-6、TNF-α是CVA的独立影响因素(OR=1.41、1.46、1.54;P_(均)<0.001)。ROC曲线表明,血清miR-223对CVA诊断价值的AUC(95%CI)为0.854(0.742~0.949),截断值为1.936,灵敏度87.0%,特异度77.8%。结论血清miR-223水平在CVA患者中明显升高,与免疫细胞及炎症指标显著相关,是CVA的独立危险因素,对CVA有较高的诊断价值。
Objective To investigate the level of miR-223 in children with cough against asthma(CVA),its correlation with inflammation and immunity and its diagnostic value.Methods A total of 53 children with cough variant asthma admitted to Hainan Modern Women and Children's Hospital from September 2020 to November 2022 were selected as the observation group,and 53 healthy children in the outpatient department were selected as the control group.The levels of micro RNA(miR)-223,T helper(Th)1,Th2,Th1/Th2,interleukin(IL)-2,IL-6 and tumor necrosis factor(TNF)-αwere measured.Results The levels of miR-223 in the observation group were higher than those in the control group,while the levels of Th1,Th2 and Th1/Th2 were lower than those in the control group(P<0.05).The levels of IL-2 in the observation group were lower than those in the control group,and the levels of IL-6 and TNF-αwere higher than those in the control group(P<0.05).Serum miR-223 levels were negatively correlated with Th1,Th2,Th1/Th2 and IL-2 levels(r=-0.675,-0.550,-0.565,-0.618;P<0.001),were positively correlated with IL-6 and TNF-α(r=0.686,0.706;P<0.001).Multivariate Logistic regression showed that miR-223,IL-6 and TNF-αwere independent influencing factors of CVA(OR=1.41,1.46,1.54;P<0.001).ROC curve showed that the AUC(95%CI)of serum miR-223 for the diagnosis of CVA was 0.854(0.742-0.949),the cut-off value was 1.936,the sensitivity was 87.0%,and the specificity was 77.8%.Conclusion Serum miR-223 level is significantly increased in patients with CVA,which is significantly correlated with immune cells and inflammatory indicators.It is an independent risk factor for CVA,and has high diagnostic value for CVA.
