宫腔镜子宫内膜息肉切除术后影响息肉复发的多因素分析:手术时机的研究 被引量:5

Multivariate Analysis of Factors Affecting Endometrial Polyp Recurrence After Transcervical Resection of Polyp: a Study on Surgical Timing

摘要 目的 探讨在黄体期行宫腔镜子宫内膜息肉切除术(transcervical resection of polyp, TCRP)是否为术后子宫内膜息肉(endometrial polyp, EP)复发的影响因素。方法 回顾性分析2018年6月~2019年6月582例TCRP的育龄期EP患者资料,术后随访12~24个月。以年龄、体重、异常子宫出血、不孕症、孕次、产次、合并子宫腺肌症、合并子宫肌瘤、合并剖宫产瘢痕憩室、子宫内膜厚度、息肉数目、联合负压吸宫术、手术时机为自变量,对EP术后复发的影响因素进行多因素分析。结果 582例中,术后EP复发45例(7.7%)(病例组),无复发537例(对照组)。255例于黄体期施术,术后16例(6.3%)复发;327例于卵泡期施术,术后29例(8.9%)复发。黄体期施术不是影响EP术后复发的因素(未校正OR=0.688,95%CI:0.365~1.296,P=0.247;校正OR=0.724,95%CI:0.372~1.411,P=0.343)。TCRP术后EP复发的预后因素有年龄(OR=1.085,P=0.008)和原发不孕(OR=3.725,P=0.000)。结论 黄体期施术不是影响TCRP术后EP复发的预后因素;年龄和原发不孕是影响TCRP术后EP复发的预后因素。 Objective To investigate whether transcervical resection of polyp(TCRP)in luteal phase is an influential factor for the recurrence of endometrial polyp(EP).Methods A total of 582 EP patients of childbearing age receiving TCRP from June 2018 to June 2019 were retrospectively analyzed.The patients were followed up for 12-24 months.The age,weight,abnormal uterine bleeding,infertility,pregnancy times,delivery times,adenomyosis,uterine fibroids,caesarean section cicatricial diverticulum,endometrial thickness,number of polyps,negative pressure aspiration,and operation time were used as independent variables to analyze the influencing factors of EP recurrence by multivariate analysis.Results Of the 582 patients,there were 45 cases(7.7%)of postoperative EP recurrence(case group)and 537 cases without recurrence(control group).A total of 255 cases underwent surgery during the luteal phase,and 16 cases(6.3%)experienced recurrence after surgery;327 cases underwent surgery during the follicular phase,and 29 cases(8.9%)experienced recurrence after surgery.Surgery performance in the luteal phase was not a prognostic factor for postoperative recurrence of EP(unadjusted OR=0.688,95%CI:0.365-1.296,P=0.247;adjusted OR=0.724,95%CI:0.372-1.411,P=0.343).The prognostic factors of EP recurrence after TCRP were age(OR=1.085,P=0.008)and primary infertility(OR=3.725,P=0.000).Conclusions Luteal phase surgery is not a prognostic factor for EP recurrence after TCRP.Age and primary infertility are prognostic factors for EP recurrence after TCRP.
