静脉全血C反应蛋白检测系统与静脉血清C反应蛋白检测系统分析结果一致性评价

摘要 目的:评估静脉全血C反应蛋白(total blood C-reactive protein,TCRP)检测系统与SCRP(serum C-reactive protein,SCRP)检测系统结果一致性,为临床提供快速、准确的CRP检验结果,真正实现缩短CRP检测的周转时间(turnaroundtime,TAT)。方法:研究对象为阳医院2022年9月23日-2022年9月28日上午7:45~9:00期间从临床病区送至检验科样本,对患者同时具有SCRP和血常规医属的,分别进行SCRP和TCRP测定。以统计学方法D’Agostino-Pearson检验数据是否呈正态分布。不同检测系统CRP间相关分析采用Spearman相关一致性分析采用Bland-Altman和Passing-Bablok回归分析。通过绘制TCRP受试者工作特征曲线(receiver operating characteristic curve,ROC),以预测SCRP阳性与否,分别CRP以≥6.0mg/L、>8.0mg/L、>10.0mg/L判定为阳性,以SCRP检测系统为参考方法。结果:TCRP检测结果略低于SCRP,但TCRP和SCRP高度相关(r=0.992(95%CI:0.990-0.993),P<0.0001);Bland-Altman图结果表明在SCRP低于50mg/L时,二者有极高的一致。Passing-Bablok回归分析得出回归方程为SCRP=1.066*TCRP-0.118;通过ROC曲线分析显示,敏感性和特异性均大于97%,以TCRP最佳阅值5.17mg/L和9.7mg/L分别预测SCRP大于6.0mg和10mg/L时,敏感性都是100%,特异性都是97%;以TCRP最佳阈值7.88mg/L和9.7mg/L分别预测SCRP天于8.0mg和10mg/L时,ROC曲线下面积均为1。结论:TCRP检测系统与SCRP检测系统高度一致性,TCRP检测系统可以满足临床检测需要,与血常规同步检测出报告为医学检验科真正实现快速的、准确的CRP检测提供了证据,确实实现了CRP检测TAT的缩短。 Objective:To evaluate the consistency of venous total blood C-reactive protein(TCRP)detection system and venous serum CRP(SCRP)detection system,to provide fast and accurate CRP test results,and to shorten the turn around time(TAT)of CRP detection.Methods:The research subjects were from Shuyang Hospital.SCRP and TCRP assays were performed.The D'Agostino-Pearson statistical method was used to test whether the data were normally distributed.Correlation analysis between CRP of different detection systems was performed by Spearman correlation,and consistency analysis was performed by Bland-Altman and Passing-Bablok regression analysis.The receiver operating characteristic curve(ROC)of TCRP was drawn to predict whether the venous SCRP was positive or not.Taking the SCRP detection system as the reference method.TCRP is represented by TCRP,and venous SCRP is represented by SCRP.Results:The detection result of TCRP was slightly lower than that of SCRP,but TCRP and SCRP were highly correlated(r=0.992(95%CI:0.990-0.993),P<0.0001);Bland-Altman plot showed that when SCRP was lower than 50 mg/L,there is a very high agreement.Passing-Bablok regression analysis showed that the regression equation was SCRP=1.066*TCRP-0.118;The ROC curve analysis showed that the sensitivity and specificity were both greater than 97%,and the optimal thresholds of TCRP were 5.17 mg/L and 9.7 mg/L,respectively.To predict the SCRP was more than 6.0 mg/L and 10 mg/L,respectively.The sensitivity was 100%,and the specificity was 97%.The optimal thresholds of 7.88 mg/L and 9.7 mg/L,respectively.To predict the SCRP was more than 8.0 mg/L and 10 mg/L,respectively.The area under the ROC curve is 1.Conclusion:The TCRP detection system are highly consistent with the venous SCRP detection system.The TCRP detection system can be satisfied the needs of clinical detection,and the synchronous detection report with the blood routine can provide the medical laboratory with a real fast and accurate CRP detection.Based on the evidence,the shortening ofTAT detected by C
