摘要
卵巢早衰(premature ovarian failure,POF)是指女性在40岁前卵巢功能衰竭,以闭经伴高促性腺激素和低雌激素为特征,其诊断标准为:闭经时间超过4~6个月、间隔>4周连续2次测出卵泡刺激素(follicle-stimulating hormone,FSH)>40 U/L。POF影响卵母细胞的数量和质量,导致女性生育能力减退甚至丧失,危害女性健康。导致POF的原因不明,可能与多种因素有关,如遗传性因素、环境因素、年龄因素、自身免疫因素、医源性因素等,其中自身免疫因素作用于该疾病的机制错综复杂,且影响日益受人们的重视。因此,本文将POF涉及的自身免疫因素整理如下,为临床上该疾病的预防和治疗提供诊疗思路。
Premature ovarian failure refers to the cessation of ovarian function in women before the age of 40.It is characterized by amenorrhea with a high gonadotropin level and a low estrogen level.The diagnostic criteria are as below:the duration of amenorrhea is more than 4-6 months;two consecutive tests with the interval of more than 4 weeks showed FSH>40 U/L.Premature ovarian failure affects the quantity and quality of oocytes,leading to the decline or even loss of female fertility,and endangers women's health.However,the cause of premature ovarian failure is unknown and may be related to a variety of factors,such as genetic factors,environmental factors,age factors,immune factors,iatrogenic factors,etc.Among them,the immune factors have a complicated mechanism acting on this disease.Therefore,the immune factors involved in premature ovarian failure are summarized in this paper to provide diagnosis and treatment ideas for its clinical prevention and treatment.
