摘要
目的分析慢性阻塞性肺疾病(COPD)患者血清Clara细胞蛋白-16(CC-16)和单核细胞趋化蛋白-1(MCP-1)的表达与病情严重程度的相关性。方法回顾性收集2021年1月至2022年12月期间辽宁省健康产业集团阜新矿总医院收治的80例COPD患者作为研究对象,根据入院时动脉血氧分压(PaO_(2))的不同,分为A组:轻度或无缺氧组(PaO_(2)>60 mmHg,n=31)、B组:中度缺氧组(40 mmHg<PaO_(2)<60 mmHg,n=28)和C组:重度缺氧组(PaO_(2)<40 mmHg,n=21)。比较3组患者的PaO_(2)、动脉血二氧化碳分压(PaCO_(2))及肺功能指标[用力肺活量(FVC)、第1秒用力呼气容积(FEV1)],采用酶联免疫吸附试验检测血清CC-16和MCP-1水平。分析COPD患者血清CC-16、MCP-1水平与病情严重程度的关系,并采用多元Logistic回归分析分析血清CC-16和MCP-1水平对COPD患者病情严重程度的影响。结果3组患者血气分析指标、肺功能指标及血清CC-16、MCP-1水平比较,差异均有统计学意义(P<0.05);B组和C组患者的PaO_(2)、FVC、FEV1及血清CC-16水平显著低于A组,PaCO_(2)和血清MCP-1水平显著高于A组,差异均有统计学意义(P<0.05);C组患者PaO_(2)、FVC、FEV1及血清CC-16水平显著低于B组,PaCO_(2)和血清MCP-1水平显著高于B组,差异均有统计学意义(P<0.05)。Pearson相关性分析显示,COPD患者血清CC-16水平与病情严重程度呈明显负相关(P<0.05),MCP-1水平与COPD病情严重程度得分呈显著正相关(P<0.05)。多元Logistic回归分析显示,血清CC-16(OR=1.255,95%CI:1.118~1.409)和MCP-1(OR=2.421,95%CI:1.311~4.470)水平均为影响COPD患者病情严重程度的独立危险因素。结论COPD患者血清CC-16和MCP-1水平与病情严重程度密切相关,可作为早期诊断和评估病情的重要生物学指标。
Objective To analysis of correlation between expression levels of serum Clara cell protein-16(CC-16)and monocyte chemoattractant protein-1(MCP-1)with severity of chronic obstructive pulmonary disease(COPD).Methods Eighty patients with COPD admitted to Fuxin General Hospital of Liaoning Health Industry Group from January 2021 to December 2022 were accepted as research subjects,and were divided into groups according to the arterial partial presure of oxpygen(PaO_(2))group A:mild or no hypoxia group(PaO_(2)>60 mmHg,n=31),group B:moderate hypoxia group(40 mmHg<PaO_(2)<60 mmHg,n=28)and group C:severe hypoxia group(PaO_(2)<40 mmHg,n=21).The PaO_(2),arterial partial presure of carbon dioxide(PaCO_(2))and pulmonary function indexes[forced vital capacity(FVC),forced expiratory volume in 1 second(FEV1)]were compared,the expression levels of serum CC-16 and MCCP-1 were detected by enzyme-linked iminunoadsordent assay(ELISA).The relationship between the levels of serum CC-16 and MCP-1 in COPD patients and the severity of the condition was analyzed.And multiple Logistic regression analysis was used to analyze the impact of serum CC-16 and MCP-1 on the severity of COPD patients'condition.Results The PaO_(2),PaCO_(2),pulmonary function indexes and expression levels of serum CC-16 and MCP-1 of three groups were statistically significant different(P<0.05);the PaO_(2),FVC,FEV1 and expression levels of serum CC-16 of group B and C were significantly lower than group A,while the PaCO_(2)and expression levels of serum MCP-1 were higher than group A,the differences were statistically significant(P<0.05);the PaO_(2),FVC,FEV1 and CC-16 of group C were lower than group B,while the PaCO_(2)and expression levels of serum MCP-1 were higher than group B,the differences were statistically significant(P<0.05).The Pearson correlation analysis showed that the expression levels of serum CC-16 were negatively correlated with severity of COPD(P<0.05),and MCP-1 were positively correlated with severity of COPD(P<0.05).Multiple linear regression analy
