摘要
目的:探讨阶段—主题匹配式教育干预在老年髋部骨折患者围手术期中的应用效果,为临床干预方式选择提供依据。方法:回顾性分析2019年6月—2022年1月上饶市人民医院收治的77例老年髋部骨折患者的病历资料,根据干预方式不同分为对照组36例和观察组41例。对照组采用常规干预,观察组采用阶段—主题匹配式教育干预。比较两组患者临床指标(首次下地时间、住院天数、健康知识掌握情况)、髋关节功能、生活能力[日常生活活动能力(Barthel指数)评分]、心理状况[老年抑郁量表(GDS)评分]、疼痛情况[数字疼痛评分法(NPRS)评分]及并发症发生率。结果:观察组患者首次下地时间及住院时间均短于对照组;干预后,观察组健康知识掌握情况评分高于对照组,差异有统计学意义(t=4.875、5.580、9.968,P<0.05)。观察组髋关节功能恢复优良率明显高于对照组,差异有统计学意义(χ^(2)=4.185,P<0.05)。干预后,观察组Barthel评分高于对照组,GDS、NPRS评分低于对照组,差异有统计学意义(t=7.322、6.093、15.935,P<0.05)。观察组并发症发生率明显低于对照组,差异有统计学意义(χ^(2)=4.556,P<0.05)。结论:在老年髋部骨折患者围手术期中应用阶段—主题匹配式教育干预,能够改善患者临床指标及髋关节功能,缓解疼痛,降低并发症发生率,提高患者生活质量。
Objective:To explore the application effect of stage-topic matched educational intervention in the perioperative pe⁃riod of elderly hip fracture patients,and to provide a basis for the choice of clinical intervention.Methods:The medical records of 77 cases of elderly hip fracture patients admitted to the hospital from June 2019 to January 2022 were restropectively analyzed and divided into 36 cases in the control group and 41 cases in the observation group according to the different modes of intervention.The control group used conventional interventions and the observation group used stage-topic matched educational interventions.Clinical indexes(first time off the floor,hospitalization days,health knowledge acquisition),hip function,living ability(activities of daily living[Barthel Index]score),psychological condition(Geriatric Depression Scale[GDS]score),pain(Numeric Pain Rating Scale[NPRS]score),and complication rate were compared between the two groups.Results:The observation group’s first time off the floor and hospitalization time were shorter than that of the control group,and after the intervention,the observation group’s health knowledge acquisition score was higher than that of the control group,and the difference was statistically significant(t=4.875,5.580,9.968,P<0.05).The excellent rate of hip function recovery in the observation group was significantly higher than that in the control group,and the difference was statistically significant(χ^(2)=4.185,P<0.05).After the intervention,the Barthel score of the observation group was higher than that of the control group,and the GDS and NPRS scores were lower than that of the control group,and the difference was statistically significant(t=7.322,6.093,15.935,P<0.05).The complication rate of the obser⁃vation group was significantly lower than that of the control group,and the difference was statistically significant(χ^(2)=4.556,P<0.05).Conclusion:Applying stage-topic-matched educational interventions in the perioperative period of elderly hip fractu
