摘要
目的探讨多巴酚丁胺联合小剂量多巴胺治疗小儿重症肺炎的临床效果。方法抽取2021年1月至2022年12月郑州大学第一附属医院收治的重症肺炎患儿80例,依据给药方案分为单药组和联合用药组,每组40例。单药组患儿给予多巴酚丁胺单药治疗,联合用药组患儿给予小剂量多巴胺联合多巴酚丁胺治疗。比较两组患儿的临床症状消失时间、肺功能指标、炎症指标及T淋巴细胞亚群水平。结果联合用药组患儿咳嗽、发热、呼吸困难及啰音等临床症状消失时间短于单药组(P<0.05)。治疗后,联合用药组肺功能指标(用力肺活量、第1秒用力呼气容积占用力肺活量的百分比、呼气流量峰值)、T淋巴细胞亚群(CD4^(+)、CD4^(+)/CD8^(+))水平均高于单药组(P均<0.05)。治疗后,联合用药组患儿炎症指标(C反应蛋白、白细胞介素6及降钙素原)水平低于单药组(P<0.05)。结论重症肺炎患儿应用多巴酚丁胺与小剂量多巴胺联合治疗可有效缩短临床症状持续时间,有效提升肺功能,缓解全身炎症反应,并提高机体细胞免疫水平。
Objective To investigate the clinical effect of dobutamine combined with low-dose dopamine on severe pneumonia in children.Methods A total of 80 children with severe pneumonia admitted to the First Affiliated Hospital of Zhengzhou University from January 2021 to December 2022 were selected,and they were divided into single drug group and combined drug group according to the administration plan,with 40 cases in each group.The single drug group was treated with dobutamine alone,and the combined drug group were treated with low-dose dopamine combined with dobutamine.The disappearance time of clinical symptoms,lung function indexes,inflammatory factors and T lymphocyte subsets of the two groups were compared.Results The disappearance time of clinical symptoms,such as cough,fever,dyspnea and rales,in the combined drug group were shorter than those in the single drug group(P<0.05).After treatment,the lung function indexes(forced vital capacity,percentage of forced expiratory volume in one second to forced vital capacity,peak expiratory flow),and levels of T lymphocyte subsets,including cluster of differentiation 4^(+)(CD4^(+))and CD4^(+)/cluster of differentiation 8^(+),in the combined drug group were higher than those in the single drug group(all P<0.05).After treatment,the inflammatory factors(C-reactive protein,interleukin-6,procalcitonin)in the combined drug group were significantly lower than those in the single drug group(P<0.05).Conclusions For children with severe pneumonia,dobutamine combined with low-dose dopamine can effectively shorten the duration of clinical symptoms,effectively improve lung function,alleviate systemic inflammation,and improve cellular immunity.
