摘要
目的探讨青年应力性L_(5)椎体峡部裂与相应节段椎间盘退变的关系。方法比较56例青年应力性L_(5)椎体峡部裂患者(峡部裂组)与同期56名青年健康志愿者(对照组)L_(5)~S_(1)椎间盘退变程度的差异性。并将峡部裂组患者按照病程<1年、1~3年、>3年分为A组(26例)、B组(17例)、C组(13例),比较3组L_(5)~S_(1)椎间盘退变程度的差异性。结果根据改良Pfirrmann分级系统评估椎间盘退变情况的评分,峡部裂组高于对照组(P<0.05)。A、B、C 3组的L_(5)~S_(1)椎间盘退变程度资料进行独立样本非参数检验结果显示,3组之间L_(5)~S_(1)椎间盘退变程度仍存在差异,A组与B组、B组与C组之间比较差异均无统计学意义(P>0.05),A组低于C组(P<0.05)。结论青年应力性L_(5)峡部裂与相应节段椎间盘退变程度相关,随着病程的延长,达到一定时间后,椎间盘退变程度会相应增加。
Objective To investigate the relationship between stress L_(5) spondylolysis and intervertebral disc degeneration of corresponding segment in the youth.Methods The difference of degree of degeneration of L_(5)~S_(1) intervertebral disc between 56 young patients with stress L_(5) spondylolysis(spondylolysis group)and 56 young healthy volunteers(control group)was compared.The patients of spondylolysis group were divided into groups A(26 cases),B(17 cases)and C(13 cases),according to the disease course<1 year,1~3 years and>3 years,and the difference of the degree of L_(5)~S_(1) lumbar disc degeneration was compared among the three groups.Results According to scores of disc degeneration assessed with the modified Pfirrmann grading system,the spondylolysis group was higher than the control group(P<0.05).The independent sample non-parametric test was used to detect the L_(5)~S_(1) disc degeneration degree of group A,B and C,and the results showed that there were still differences in the L_(5)~S_(1) disc degeneration degree among the three groups,and there were no statistical differences between group A,B,and between group B,C(P>0.05),group A was lower than group C(P<0.05).Conclusions There is a correlation between the spondylolysis of L_(5) and intervertebral disc degeneration of corresponding segment in the youth,with the prolongation of the course of the disease,the degree of disc degeneration will increase correspondingly after a certain period of time.
