摘要
目的:通过文献对透析后疲乏的替代术语、定义属性、前因变量、后果、相关概念及典型案例进行系统分析,以期为护理实践提供参考。方法:检索Pub Med、Web of Science、the Cochrane Library、CINAHL、中国生物医学文献数据库、中国知网、维普期刊网和万方数据库,检索时限为建库至2022年10月31日。根据纳入标准选取50篇文献,以Rodgers演化概念分析法为框架进行分析。结果:透析后疲乏涉及4个定义性属性,包括个体的主观体验并具有客观表现、不愉快的症状体验、多维疲乏体验及透析后间歇性疲乏状态。相关因素包括治疗相关因素、个人因素、实验室检查;结局指标包括不良事件风险增加、限制生活参与、心理及认知障碍。结论:透析后疲乏的概念在不断更新和完善,对透析后疲乏的概念分析有助于医护人员构建行之有效的疲乏管理策略,以降低透析后疲乏水平,提高病人生活质量。
Objective:To systematically analyze the alternative terms,defining attributes,antecedent variables,consequences,related concepts and typical cases of Post dialysis fatigue through literature,in order to provide reference for nursing practice.Method:PubMed,Web of Science,the Cochrane Library,CINAHL,China Biology Medicine Disc,China National Knowledge Infrastructure(CNKI),VIP Journal Network and Wanfang Database were searched from the establishment of the databases to October 312022.According to the inclusion criteria,50 literature were selected and analyzed with the Rodgers evolution concept analysis method.Results:Post dialysis fatigue involved four defining attributes,individual subjective experience with objective performance,unpleasant symptom experience,multi⁃dimensional fatigue experience and intermittent fatigue state after dialysis.The related factors included treatment related factors,personal factors and laboratory tests.Outcome measures included increased risk of adverse events,limitations in life participation,psychological and cognitive impairment.Conlusion:The concept of post dialysis fatigue is constantly being updated and refined.The analysis of the concept of post dialysis fatigue is helpful for medical staff to build effective fatigue management strategies,reduce the level of post dialysis fatigue and improve the quality of life of patients.
