摘要
目的调查慢性阻塞性肺疾病(COPD)患者症状体验、衰弱情况以及照顾者照顾负担水平,并分析三者之间的关系。方法采用横断面调查方法,选取2020年2月至2022年12月安徽临泉县人民医院收治的100例COPD患者及其照顾者,采用一般资料调查表、Tilburg衰弱量表、记忆症状评估量表、Zarit照顾者负担量表对其进行调查。采用多元线性回归分析COPD患者衰弱的影响因素,通过建立结构方程模型评价照顾者照顾负担在症状体验和衰弱之间的中介效应。结果研究有效回收问卷97份,COPD患者衰弱总分为(5.12±1.35)分,症状体验总分为(1.41±0.52)分,照顾负担总分为(26.72±6.53)分。COPD患者衰弱得分与症状体验得分和照顾者照顾负担得分呈正相关(r=0.367~0.891,P<0.01);症状体验得分与照顾者照顾负担得分呈正相关(r=0.306~0.969,P<0.01)。症状体验和照顾者照顾负担得分是影响COPD患者衰弱的主要危险因素(均P<0.05),共解释总变异的50.8%。症状体验对衰弱的直接效应值为0.92(95%CI:0.846~1.044),通过照顾者照顾负担所发挥的中介效应值为0.11(95%CI:0.190~0.791),中介效应占总效应的10.7%。结论照顾者照顾负担在COPD患者症状体验和衰弱之间起中介作用,医护人员应通过降低照顾者照顾负担水平,减轻症状体验对患者衰弱的影响。
Objective To investigate the levels of symptom experience,frailty and caregiver care burden in patients with chronic obstructive pulmonary disease(COPD),and to analyze their correlation.Methods It was a cross-sectional study involving a total of 100 COPD patients treated in the L.Q.People’s Hospital from February 2020 to December 2022 and corresponding caregivers.They were investigated by general information questionnaire,the Tilburg Frailty Indicator(TFI),the Memorial Symptom Assessment Scale(MSAS),and the Zarit Burden Interview(ZBI).Multiple linear regression was conducted to analyze the influencing factors for the frailty of COPD patients,and a structural equation model was established to evaluate the mediation effect of caregiver care burden between symptom experience and frailty.Results A total of 97 valid questionnaires were collected in the present study.The scores of frailty and symptom experience in COPD patients,and caring burden in caregiver were(5.12±1.35)points,(1.41±0.52)points and(26.72±6.53)points,respectively.The scores of frailty was positively correlated with those of symptom experience and caregiver care burden(r=0.367-0.891,P<0.01),and the scores of symptom experience was positively correlated with those of caregiver care burden(r=0.306-0.969,P<0.01).Symptom experience and caregiver care burden scores were the main risk factors for the frailty of COPD patients(both P<0.05),accounting for 50.8%of the total variance.The direct effect value of symptom experience on the frailty of COPD patients was 0.92(95%CI:0.846-1.044).The mediating effect value of caregiver care burden was 0.11(95%CI:0.190-0.791),accounting for 10.7%of the total effect.Conclusion The caregiver care burden plays a mediating role between the symptom experience and frailty of COPD patients.Medical staff should alleviate the caregiver care burden,so as to reduce the effect of symptom experience on frailty of COPD patients.
