克氏针张力带钢丝联合锚钉治疗髌骨下极骨折的效果分析 被引量:2

摘要 目的:探讨克氏针张力带钢丝联合锚钉治疗髌骨下极骨折和修复髌韧带的临床疗效。方法:回顾性分析2016年1月至2021年12月采用克氏针张力带钢丝联合锚钉治疗的29例髌骨下极骨折患者的病例资料。其中男16例,女13例;年龄21~71岁,平均(50.7±12.2)岁。髌骨骨折部位:左侧15例,右侧14例,均为闭合性骨折。采用膝关节活动度和B?stman评分系统评价膝关节功能。结果:29例患者均顺利完成手术。平均手术时间(59.1±6.2)min,平均术中出血量(17±8)mL。术后随访3~12个月,平均随访时间(6.5±3.3)个月。未出现切口感染、骨折移位、骨折不愈合及膝关节僵硬等并发症。患侧和健侧膝关节活动度分别为128.65°±5.23°和131.78°±7.23°,两者比较差异无统计学意义(P>0.05)。Bostman评分平均(27.9±2.7)分,优19例,良10例,优良率100%。结论:克氏针张力带钢丝联合锚钉治疗髌骨下极骨折,骨折复位、固定效果好,可兼顾髌韧带修复,膝关节功能恢复良好。 Objective:To investigate the clinical outcome of Kirschner wire tension band steel wire combined with anchor screw in the fixation of patellar inferior pole fracture and the repair of injured patellar ligament.Methods:A total of 29 patients with the fracture of the inferior pole of patella treated with Kirschner wire tension band steel wire combined with anchor technique from January 2016 to December 2021 were retrospectively analyzed.There were 16 males and 13 females with a mean age of(50.7±12.2)years(range,21-71 years).All cases were closed fractures,and the left was involved in 15 cases and the right was involved in 14 cases.Range of motion(ROM)and Böstman scoring system were used to evaluate knee function after surgery.Results:The surgery was successfully completed in all the patients.The average operation time was(59.1±6.2)min and average intraoperation blood loss was(17±8)mL.The average follow-up time was(6.5±3.3)months(range,3-12 months).During the follow-up,no complications such as incision infection,fracture displacement,fracture nonunion and knee joint stiffness occurred.There was no significant difference in ROM between the affected side and the healthy side(128.65°±5.23°vs.131.78°±7.23°,P>0.05).The average Böstman score was 27.9±2.7,excellent in 19 cases,good in 10 cases,and the excellent and good rate was 100%.Conclusions:For patellar inferior pole fracture,Kirschner wire tension band steel wire combined with anchor screw technology can achieve good reduction and fixation and take into account the reconstruction and repair of patellar ligament,and the function of knee joint can be restored well.
