摘要
目的系统评价糖萼损伤生物标志物多配体聚糖-1(syndecan-1)对成人脓毒症/脓毒性休克患者预后的影响。方法以糖萼、多配体聚糖-1、脓毒症、脓毒性休克等为检索词,查找中国生物医药文献数据库(CBM)、万方数据库、中国知网(CNKI)、维普数据库等多个数据库获取相关研究文献;在包括德国施普林格全文数据库(Springer Link)、美国国家医学图书馆PubMed数据库、荷兰医学文摘、科学网、Cochrane图书馆在内的多个数据库中,以glycocalyx、syndecan-1、SDC-1、sepsis、severe sepsis、septic shock等为搜索关键词,获取相应的研究文献。检索关于糖萼损伤生物标志物syndecan-1水平对成人脓毒症/脓毒性休克患者预后影响的研究,检索时限为2000年1月至2023年6月。由2名研究人员分别进行文献筛选、数据提取和质量评价,应用Rev Man 5.2软件对符合标准的文献进行Meta分析。结果共纳入10篇文献,共有患者1297例,其中死亡组336例,存活组961例。Meta分析结果显示,成人脓毒症/脓毒性休克患者糖萼损伤生物标志物syndecan-1水平增高与患者死亡[标准化均差(SMD)=0.75,95%CI 0.42~1.08,Z=4.51,P<0.00001]、合并急性肾损伤(AKI)(SMD=0.51,95%CI 0.37~0.66,Z=6.92,P<0.00001)、合并弥散性血管内凝血(DIC)(SMD=0.62,95%CI 0.26~0.98,Z=3.34,P=0.0008)相关。结论糖萼损伤生物标志物syndecan-1水平与成人脓毒症/脓毒性休克患者的预后有关,可能是脓毒症病死率和相关并发症的有用预测指标。
Objective To systematically evaluate the effect of glycocalyx biomarker syndecan-1 on the prognosis of adult patients with sepsis/septic shock.Methods We searched all relevant articles about the effect of glycocalyx biomarker syndecan-1 on the prognosis in adult patients with sepsis/septic shock published in CBM,Wanfang,CNKI,VIP database,Springer Link,PubMed,Embase,Web of Science,Cochrane library from January 2000 to June 2023,and with following keywords:glycocalyx,syndecan-1,SDC-1,sepsis,severe sepsis,septic shock.Literature screening,data extraction and quality evaluation were carried out by two researchers respectively,and Meta-analysis was performed by using Rev Man 5.2 software.Results Ten literatures were included,with a total of 1297 patients,336 in the death group and 961 in the survival group.Meta-analysis showed that increased levels of glycocalyx biomarker syndecan-1 in adult patients with sepsis/septic shock were associated with the death[standardized mean difference(SMD)=0.75,95%CI 0.42-1.08,Z=4.51,P<0.00001],acute kidney injury(AKI)(SMD)=0.51,95%CI 0.37-0.66,Z=6.92,P<0.00001],and disseminated intravascular coagulation(DIC)(SMD=0.62,95%CI 0.26-0.98,Z=3.34,P=0.0008).Conclusions Glycocalyx biomarker syndecan-1 is associated with the prognosis of adult sepsis/septic shock patients and may be a useful predictor of sepsis mortality and related complications.
