摘要
目的探讨Mulligan关节松动术联合龙氏治脊疗法治疗儿童外伤性寰枢关节半脱位的临床疗效。方法选择2020年1月至2021年4月于东莞市第八人民医院门诊确诊的93例外伤性寰枢关节半脱位患儿,随机分为对照组46例,观察组47例。对照组以小重量多次牵引复位和颈托固定治疗,观察组采用Mulligan技术联合龙氏治脊,2个疗程结束后,评估2组临床有效率、影像学结果、颈椎主动活动度及疼痛评分。结果治疗后,观察组临床有效率为83.0%,优于对照组的63.0%(P<0.05);观察组在影像学指标、颈椎主动活动度及疼痛评分上,均优于对照组(P<0.05)。结论Mulligan关节松动术联合龙氏治脊疗法治疗儿童外伤性寰枢关节半脱位安全有效,值得临床推广。
Objective To investigate the clinical effects of Mulligan concept combined with Long’s chiropractic on the treatment of raumatic subluxation of atlantoaxial joint in children.Methods A total of 93 pediatric patients with traumatic subluxation of atlantoaxial joint who either sought medical care in the outpatient or admitted into the hospital from January 2020 to April 2021 were selected and randomly divided into the control group and the observation group,each consisting of 46 and 47 patients.The control group received light weight multiple traction reduction therapy,while the observation group was treated with a combination of Mulligan concept and Long's chiropractic therapy.After two course of treatment,the clinical efficiency,radiological imaging data,range of motion(ROM)of the cervical spine and the scores of visual analog scale(VAS)were evaluated in the two groups.Results After treatment,the effective rate of the observation group was 83.0%,which was superior to 63.0%of the control group(P<0.05).The imaging data,the range of motion of the cervical spine and the VAS scores of the observation group were all superior to those of the control group,with clinical efficacy within short time(P<0.05).Conclusion The combination of Mulligan concept and Long's chiropractic therapy is safe and effective in the treatment of traumatic subluxation of atlantoaxial in children,therefore,it is worth further clinical promotion.
