摘要
目的 探讨无创呼吸机联合低流量吸氧对老年慢性阻塞性肺疾病(COPD)急性发作动脉血气的影响。方法 选取收治的COPD老年患者80例,按照随机信封法分为2组,对照组(n=40例,低流量吸氧疗法),观察组(n=40例,低流量吸氧疗法+无创呼吸机通气疗法),比较2组COPD患者治疗前、治疗14d后的肺功能指标、动脉血气指标以及住院指标。结果 观察组治疗14d后1秒用力呼气量(FEV1)、肺活量(VC)与50%呼气流量峰值(PEF50)高于对照组(P<0.05);观察组治疗14d后动脉血pH与对照组差异无统计学意义(P>0.05),动脉血氧分压(PaO_(2))高于对照组(P<0.05),动脉血二氧化碳分压(PaCO_(2))低于对照组(P<0.05);观察组住院总天数、ICU住院天数小于对照组(P<0.05),观察组插管率与对照组差异无统计学意义(P>0.05)。结论 低流量吸氧治疗+无创呼吸机通气用于治疗老年COPD患者能改善患者肺功能、动脉血气,减少住院天数,值得在临床进行推广。
Objective To investigate the effect of non-invasive ventilator combined with low-flow oxygen inhalation on arterial blood gas in elderly patients with acute outbreak of chronic obstructive pulmonary disease(COPD).Methods A total of 80 elderly patients with COPD were selected and divided into two groups according to random envelope method,the control group(n=40 cases,low flow oxygen therapy)and the observation group(n=40 cases,low flow oxygen therapy+non-invasive ventilator ventilation therapy).The pulmonary function indexes,arterial blood gas indexes and hospitalization indexes of COPD patients before and 14 days after treatment were compared between the two groups.Results After 14 days of treatment,the FEV1,VC and PEF50 in the observation group were higher than those in the control group(P<0.05).After 14 days of treatment,the arterial blood pH of the observation group was no different from that of the control group(P>0.05),PaO2 was higher than that of the control group(P<0.05),and PaCO2 was lower than that of the control group(P<0.05).The total days of hospitalization and ICU stay in the observation group were less than those in the control group(P<0.05),and there was no difference in the intubation rate between the observation group and the control group(P>0.05).Conclusions Low-flow oxygen therapy combined with non-invasive ventilator ventilation in the treatment of elderly COPD patients can improve lung function,arterial blood gas,and reduce hospitalization days,and it is worthy of clinical promotion.
