摘要
目的 探讨血清白细胞介素-17(IL-17)、白细胞介素-33(IL-33)及呼出一氧化氮浓度在慢性咳嗽患者诊断中的应用价值.方法 选取2017年1月至2018年4月慢性咳嗽患者127例为观察组,另选择同期体检的健康者141例为对照组.根据患者慢性咳嗽的发病类型将观察组分为咳嗽变异性哮喘组、嗜酸粒细胞性支气管炎组和其他慢性咳嗽组.比较两组患者IL-17、IL-33及呼出一氧化氮浓度(FeNO);比较观察组各分组患者呼出一氧化氮浓度水平;使用logistics多因素回归分析探究IL-17、IL-33与患者第一秒用力呼气容积占预计值的比值(FEV1%)的相关性;使用ROC曲线对FeNO诊断慢性咳嗽的敏感度和特异度进行分析.结果 观察组患者IL-17(64.36±8.92)ρB/(ng·L)、IL-33(58.74±11.20)ρB/(ng·L)及FeNO水平(36.68±7.26)ppb均高于对照组患者(27.69±9.44)ρB/(ng·L)、(30.17±10.48)ρB/(ng·L)、(15.20±8.22)ppb,两组比较差异有统计学意义(P<0.05).FeNO水平比较,咳嗽变异性哮喘组(57.82±12.56)ppb高于嗜酸粒细胞性支气管炎组(32.08±9.49)ppb和其他慢性咳嗽组(21.76±10.13)ppb,三组比较差异有统计学意义(P<0.05).ROC曲线显示FeNO诊断慢性咳嗽的敏感度和特异度较高(P<0.05).结论 血清IL-17、IL-33以及FeNO对于诊断慢性咳嗽具有良好的临床应用价值,可对以上指标进行监测以提高慢性咳嗽的诊出率.
Objective To explore the value of serum interleukin-17,interleukin-33 and exhaled nitric oxide in the diagnosis of chronic cough.Methods 127 patients with chronic cough who were treated in our hospital from January 2017 to April 2018 were selected as study objects.Among them,chronic cough patients were used as observation group,and 141 healthy individuals who were admitted to the hospital for physical examination at the same time were used as the control group.According to the type of chronic cough in patients,the components observed were cough variant asthma group,eosinophilic bronchitis group and other chronic cough group.The levels of interleukin-17(IL-17),interleukin-33(IL-33)and exhaled nitric oxide(FeNO)were compared between the two groups.The levels of IL-17,IL-33 and exhaled nitric oxide(FeNO)were compared between the two groups.The correlation between IL-17,IL-33 and the ratio of forced expiratory volume in the first second(FEV1%)was explored by logistic multivariate regression analysis.The sensitivity to FeNO in the diagnosis of chronic cough using ROC curve degree and specificity were analyzed.Results The levels of IL-17(64.36±8.92[ρB/(ng·L)])IL-33(58.74±11.20[ρB/(ng·L)])and FeNO(36.68±7.26ppb)in the observation group were significantly higher than those in the control group(27.69±9.44[ρB/(ng·L)]30.17±10.48[ρB/(ng·L)]15.20±8.22ppb(P<0.05).The comparison of FeNO level showed that,Cough variant asthma group(57.82±12.56ppb)was higher than eosinophil bronchitis group(32.08±9.49ppb)and other chronic cough group(21.76±10.13ppb).There was significant difference between the three groups(P<0.05).Data comparison showed that the sensitivity and specificity of FeNO in the diagnosis of chronic cough was higher than that of FeNO(P<0.05.ROC).(P<0.05).Conclusion Serum IL-17,IL-33 and FeNO have a good clinical value in the diagnosis of chronic cough,which can be monitored to improve the diagnosis rate of chronic cough.
