摘要
目的回顾性分析天津某综合医院死亡病例资料,了解其死因构成、分布特点及变化趋势,为制定疾病防治措施提供依据。方法提取中国疾病预防控制信息系统中某院2009年1月1日-2018年12月31日的死亡病例资料,根据国际疾病分类标准ICD10对根本死因统一编码,对10年间死因分布情况进行描述性分析。结果10年共计报告12249例死亡病例,死因顺位前5位疾病为循环系统疾病、肿瘤、呼吸系统疾病、损伤和中毒、消化系统疾病。前5位死因在各年份间构成存在差异(P=0.000),近年来循环系统疾病死因构成有所上升。男性与女性死亡比为1.70:1,不同性别前5位死因顺位一致,男性损伤与中毒死因构成明显高于女性(10.30%与4.50%)。各年龄组死因顺位构成存在差异(P=0.000),死亡人数以60岁以上组最多(70.84%),以循环系统疾病死因为主,5~14岁组占比最少(0.34%),以先天畸形、变形和染色体异常死因为主。结论循环系统疾病为某院收治患者的首要死因,总体呈上升趋势,以中老年人群为主。医院应根据死因谱特点持续加强健康促进工作,尤其需重点关注循环系统疾病的防治,提升人群健康素养,提高生存质量。
Objectives This study aims to retrospectively analyze the death cases in a general hospital in Tianjin and understand the composition,distribution characteristics,and changing trends of the causes of death,so as to provide a basis for formulating disease prevention and control measures.Methods The death case data of a hospital from January 1,2009 to December 31,2018 were extracted from the China Disease Prevention and Control Information System.The underlying cause of death was uniformly coded according to the International Classification of Diseases standard ICD10.A descriptive analysis was performed on the distribution of causes of death in 10 years.Results A total of 12,249 deaths were reported in ten years.The top five causes of death were cardiovascular disease,tumors,respiratory diseases,injuries and poisoning,and gastrointestinal disease.The top five causes of death were different between years(P=0.000),and the cause of death from circulatory diseases had increased in recent years.The ratio of male to female was 1.70:1.The order of the top 5 causes of death of different genders was the same.The causes of injury and poisoning in males were significantly higher than that in females(10.30%vs 4.50%).There were differences in the composition of the cause of death in each age group(P=0.000).The number of deaths was the most in the group over 60 years old(70.84%),and the main cause of death was circulatory system diseases.The 5-to-14-year-old group accounted for the least(0.34%),and the main causes of death were congenital malformations,deformations,and chromosomal abnormalities.Conclusions Cardiovascular disease was the leading cause of death among patients admitted to a hospital,and the overall trend was on the rise,mainly among the middle-aged and elderly population.Hospitals should continue to strengthen health promotion work according to the characteristics of the cause of death spectrum,especially focusing on the prevention and treatment of circulatory system diseases,improving the health literacy of the po
