摘要
目的探讨脉康宁对脾肾气虚兼血瘀证血液透析患者的动静脉内瘘的临床疗效。方法选取2021年7月1日至2022年10月31日福建中医药大学附属宁德中医院血透室收治的70例行血液透析治疗的脾肾气虚兼血瘀证患者为研究对象,采用随机数字表法分为对照组(35例)和研究组(35例)。对照组采用常规治疗、常规的内瘘护理及锻炼,研究组为常规治疗加脉康宁治疗。比较两组治疗前和治疗2个月的中医症状积分、吻合口近心端血管内径及远心端血管内径、血液透析中泵控血流量、内瘘血流量,以及两组血液透析并发症发生率。结果治疗2个月后,两组的中医症状积分显著降低,吻合口近心端及远心端内径显著增大,血液透析中泵控血流量、内瘘血流量显著提高,且研究组各项指标改善幅度均大于对照组(均P<0.05)。在比较血液透析并发症发生率中,两组比较无统计学差异(P>0.05)。结论脉康宁可缓解血液透析患者血瘀症状,扩张血管,改善内瘘局部血流动力学,促进内瘘成熟,还减少血液透析并发症的发生。
Objective To investigate the clinical effects of Maikangning on the internal arteriovenous fistula of patients with spleen and kidney qi deficiency combined with blood stasis syndrome.Methods 70 patients with spleen and kidney qi deficiency combined with blood stasis syndrome undergoing hemodialysis treatment admitted to Ningde Hospital of Traditional Chinese Medicine Affiliated to Fujian University of Chinese Medicine from July 1,2021 to October 31,2022 were selected as the research objects.Random number table method was used to divide the control group(35 cases)and study group(35 cases).The control group was treated with conventional treatment,routine internal fistula nursing and exercise,while the study group was treated with conventional treatment plus pulse corning therapy.The scores of TCM symptoms,the inner diameter of proximal and distal anastomotic ends,pump controlled blood flow and internal fistula blood flow during hemodialysis were compared between the two groups before treatment and 2 months after treatment,and the incidence of hemodialysis complications was statistically and compared between the two groups.Results After 2 months of treatment,the traditional Chinese medicine symptom score of the two groups was significantly reduced,the inner diameter of the proximal and distal anastomotic ends was significantly increased,and the pump controlled blood flow and internal fistula blood flow in hemodialysis were significantly increased,and the improvement range of various indicators in the study group was greater than that in the control group,with statistical difference(P<0.05).There was no significant difference in the incidence of hemodialysis complications between the two groups(P>0.05).Conclusions Maikangning can relieve blood stasis symptoms in hemodialysis patients,increase the inner diameter of blood vessels,improve the local hemodynamics of internal fistula,promote the maturation of internal fistula,and reduce the incidence of hemodialysis complications.
