摘要
目的探讨发绀型先天性心脏病(cyanotic congenital heart disease,CCHD)早产儿术前营养不良的危险因素。方法本研究对2017年1月1日至2021年10月1日于广东省人民医院住院行心脏外科手术治疗的83例CCHD早产婴儿的资料进行分析。根据术前年龄别体质量百分位数,分为营养不良组和非营养不良组。对比两组患儿的临床资料,采用Logistic回归分析对患儿术前营养不良的影响因素进行分析,并绘制风险模型受试者工作特征曲线(receiver operating characteristic curve,ROC)以评估其预测价值。结果入选患儿共83例,营养不良组21例,非营养不良组62例,术前营养不良的发生率为25.3%。多因素Logistic回归分析显示小于胎龄儿、手术年龄较大为术前营养不良的独立危险因素(P<0.05)。将上述危险因素采用逐步回归法建立预测模型,ROC分析结果显示,该预测模型的曲线下面积为0.860,预测价值较高。结论CCHD早产儿术前营养不良的发生率较高。小于胎龄儿、手术年龄较大的患儿更易出现术前营养不良。
Objectives To explore the risk factors of preoperative malnutrition in premature infants with cyanotic congenital heart disease(CCHD).Methods The clinical data of 83 preterm infants with CCHD who were hospitalized and underwent cardiac surgery in Guangdong Provincial People′s Hospital from January 1,2017 to October 1,2021 were analyzed.According to the preoperative weight-for-age percentile,the subjects were divided into malnutrition group and non-malnutrition group.The clinical data of the two groups were compared.The Logistic regression was used to analyze the influencing factors of preoperative malnutrition and a risk model receiver operating characteristic curve(ROC)was drawn to evaluate its predictive value.Results The 83 infants were enrolled,including 21 cases in malnutrition group and 62 cases in non-malnutrition group.The incidence of preoperative malnutrition was 25.3%.Small for gestational age and older age at surgery were the independent risk factors of preoperative malnutrition(P<0.05).The above risk factors were used to establish a prediction model by stepwise regression method.ROC curve analysis showed that the area under the curve of the prediction model was 0.860,which present a high predictive value.Conclusions The prevalence of preoperative malnutrition in preterm infants with CCHD is high.Those with small for gestational age and older age at surgery are more prone to produce preoperative malnutrition.
