摘要
目的:探讨类似卵巢性索肿瘤的子宫肿瘤(UTROSCT)的病理及临床特点,促进临床对UTROSCT的认识。方法:回顾性分析并总结8例UTROSCT临床病理、免疫组化及分子遗传学特征,学习相关文献并总结其鉴别诊断要点。结果:8例患者年龄40~81岁,肿瘤最大直径2.0~10.0 cm,切面为实性,灰白、灰黄色。镜下肿瘤细胞呈上皮样或性索样,核分裂象少见。呈片状、巢状、小梁状或条索状排列,少数为裂隙样及肾小球样排列。免疫组化显示肿瘤细胞不同程度表达上皮(5/7)、性索(8/8)、平滑肌(6/7)、激素受体(8/8)及子宫内膜间质(5/8)标记。2例行FISH检测,均检测出NCOA1易位,均未发现JAZF1易位,1例行RNA测序,显示GREB1-NCOA1基因融合。7例患者随访时间4~96个月不等,1例复发,1例盆腔转移,其余5例均无瘤生存。结论:UTROSCT是一种罕见的子宫肿瘤,大部分呈良性经过,组织学表现、免疫表型及分子学改变有助于诊断。
Objective:To investigate the clinicopathological characteristics of uterine tumor resembling ovarian sex cord tumor(UTROSCT)and to promote the clinical understanding of UTROSCT.Methods:The clinicopathological and immunohistochemical features and genetic features of 8 cases of TUROSCT were analyzed retrospectively.The literature was reviewed and the diagnosis and differential diagnosis were summarised.Results:There were 8 patients aged 40 to 81 years old with tumors of maximum diameter of 2.0 cm to 10.0 cm,solid,grayish and grayish-yellow in section.Microscopically,the tumor cells were epithelioid or sex cord-like,and nuclear schizophrenia was rare.They were arranged in sheets,nests,trabeculae or cords,and a few of them were fissure-like and glomeruloid.Immunohistochemistry showed that the tumor cells expressed epithelial(5/7),cord(8/8),smooth muscle(6/7),hormone receptor(8/8)and endometrial mesenchymal(5/8)markers to different degrees.Two patients underwent FISH,both of which detected the NCOA1 translocation,neither of which revealed the JAZF1 translocation,and one underwent RNA sequencing,which showed a GREB1-NCOA1 gene fusion.Seven patients were followed up for periods ranging from 4 to 96 months,with one recurrence,one pelvic metastasis,and the remaining five were tumor-free survivors.Conclusion:UTROSCT is a rare uterine tumor with benign prognosis in most cases.Histological manifestations,immunophenotype and molecular changes are helpful for diagnosis.
