摘要
目的探讨胃肠道间质瘤(GIST)临床病理特征。方法回顾性分析2011年1月至2020年12月延边大学附属医院收治的GIST 138例,通过常规苏木精-伊红(HE)染色及免疫组织化学染色确诊GIST,分析临床病理资料,并文献复习。结果GIST高发年龄为50~70岁,其中男性78例,女性60例。好发部位是胃61例(44.2%),小肠53例(38.4%),其次肠系膜、大肠、食管、腹膜等部位;肿瘤长径0.3~23 cm,多为单发,多发结节占5.1%,其中20例(14.5%)发生转移,1例罕见髂骨转移;14例(10.10%)伴发其他恶性肿瘤。显微镜下,肿瘤细胞多呈梭形、上皮样形,交错、弥漫排列,免疫组织化学染色肿瘤细胞表达酪氨酸激酶受体(CD117),胃肠道蛋白1(DOG1),骨髓干细胞抗原(CD34),不同程度表达平滑肌肌动蛋白(SMA)和S-100肌钙蛋白(S-100)。结论GIST是最常见的胃肠道间叶源性肿瘤,主要发生于胃,中老年人多发,具有独特的临床、组织学表现,需掌握其临床病理学特点,以早期发现并制定正确的治疗方案。
Objective To investigate the clinicopathologic features of gastrointestinal mesenchymal stromal tumors(GIST).Methods A total of 138 cases of GIST admitted to Affilated Hospital of Yanbian University from January 2011 to December 2020 were retrospectively analyzed,and the diagnosis of GIST was confirmed by routine hematoxylin-eosin(HE)staining and immunohistochemistry staining,and clinicopathological data were analyzed and literature review was performed.Results The high prevalence age of GIST was 50~70 years old,of which 78 cases were male and 60 cases were female.The prevalent sites were stomach in 61 cases(44.2%),small intestine in 53 cases(38.4%),followed by mesentery,large intestine,esophagus,peritoneum and other sites;the tumours were 0.3~23 cm in length,most of them were single,and multiple nodules accounted for 5.1%,of which metastasis occurred in 20 cases(14.5%),and iliac metastasis was rare in 1 case;14 cases(10.1%)were accompanied by other malignant tumors.Under the microscope,the tumor cells were mostly spindle-shaped,epithelioid,interlaced and diffusely arranged,and immunohistochemical staining of the tumor cells expressed CD117,DOG1,CD34,and to varying degrees SMA and S-100.Conclusion GIST is the most common mesenchymal origin tumors of the gastrointestinal tract,which mainly occurs in the stomach,and is prevalent in middle-aged and elderly people,and has unique clinical and histological manifestations,which need to be grasped for its clinicopathological characteristics,for early detection,and for the development of correct treatment plans.
