PMCT的溺死诊断价值研究

Study on the potential application value of the postmortem computed tomographic to the diagnosis of drowning

摘要 目的对溺死组和非溺死组PMCT影像学测量数据进行统计分析,筛选出可用于诊断溺死的可靠性指标,以辅助诊断溺死。方法以175例北京水域溺死的尸体为溺死组,91例非溺死尸体为非溺死组,经全身多层螺旋CT扫描后采集影像学数据,进行对比分析。结果上颌窦积液CT值溺死组明显低于非溺死组(p<0.05);气管内积液CT值溺死组明显低于非溺死组(p<0.05);左心室液体CT值溺死组明显低于非溺死组(p<0.05);左心房液体CT值溺死组明显低于非溺死组(p<0.05);胃内容物CT值溺死组明显低于非溺死组(p<0.05)。结论溺死组上颌窦、气管内常见积液,且溺死组上颌窦积液CT值、气管内积液CT值、左心室和左心房液体CT值和胃内容物CT值明显低于非溺死组,综合分析可作为诊断溺死的可靠指标,PMCT对诊断溺死具有辅助价值。 Objective To analyze the PMCT imaging data of the drowning group and the non-drowning group,and select the reliable index that can be used to assist the diagnosis of drowning.Methods 175 drowning bodies in Beijing waters were chosen as the drowning group and 91 non-drowning bodies were selected as non-drowning group.The imaging data by multi-layer spiral CT scanning of all cases were compared and analyzed.Results The CT value of fluid in the maxillary sinus in drowning group was significantly lower than that in non-drowning group(p<0.05).The CT value of airway contents in drowning group was significantly lower than that in non-drowning group(p<0.05).The CT value of left ventricular fuid in drowning group was significantly lower than that in non-drowning group(p<0.05).The CT value of left atrial fluid in drowning group was significantly lower than that in non-drowning group(p<0.05).The CT value of gastric contents in drowning group was significantly lower than in non-drowning group(p<0.05).Conclusion There are usually fluid in the maxillary sinus and airways in drowning group.In drowning group,the CT value of the fuid in the maxillary sinus,the CT value of the airways contents,the CT value of the leff ventricular fluid and left atrial fuid,and the CT value of the gastric contents are significantly lower than those in non-drowning group,which can be comprehensively considered as a reliable index for the diagnosis of drowning.PMCT can provde assiting information for the diagnosis of drowning.
