摘要
目的探讨双相情感障碍患者病耻感、自尊、自知力与应对方式之间的关系。方法采用方便采样法选取182例双相情感障碍患者为研究对象,采用一般情况调查表、心理疾病内化污名量表、自尊量表、自知力及治疗态度问卷与简易应对方式问卷进行测评。结果不同家庭月收入双相情感障碍患者病耻感存在显著差异(P<0.05)。患者积极应对方式与病耻感、自尊水平呈显著负相关(P<0.05);消极应对方式与病耻感、自尊水平及自知力呈显著正相关(P<0.05)。中介效应检验显示,自尊在病耻感和积极应对方式之间起完全中介作用,自尊水平与自知力在病耻感和消极应对方式之间起链式中介作用。结论病耻感通过自尊水平及自知力间接影响患者应对方式,提示医护人员在干预过程中以此路径提高患者服药依从性,改善预后。
Objective To explore the relationship of stigma,self-esteem and insight with coping styles in patients with bipolar disorder.Methods Evaluations were conducted using general information questionnaire,Internalized Stigma Scale of Mental Illness(ISMI)Scale,Self-esteem Scale(SES),Insight and Treatment Attitude Questionnaire(ITAQ)and Simplified Coping Style Questionnaire(SCSQ)in 182 bipolar disorder patients selected according to convenient sampling method.Results There were significant differences in stigma among bipolar disorder patients with different family monthly incomes(P<0.05).The positive coping style of patients was negatively correlated with stigma and self-esteem(P<0.05);negative coping style was positively correlated with stigma,self-esteem and insight(P<0.05).The mediating effect test shows that self-esteem played a complete mediating role between stigma and positive coping styles,self-esteem and insight did a chain mediating role between stigma and negative coping styles.Conclusion Stigma indirectly influences patients'coping style through self-esteem and insight,suggesting that medical staff can improve patients'medication compliance and prognosis through this path in the intervention process.
