摘要
目的探讨黄芩牡丹汤加味保留灌肠联合抗生素治疗盆腔炎性疾病后遗症的效果以及血清白细胞介素(IL)-6与肿瘤坏死因子(TNF)-α水平的影响。方法选取2018年1月—2020年1月在该院就诊的盆腔炎性疾病后遗症女性患者92例。按随机数字表法分为治疗组(46例)和对照组(46例)。两组给予常规抗生素治疗措施;对照组予裸花紫珠片;治疗组予黄芩牡丹汤加味保留灌肠。两组连续治疗3个月。比较两组湿热瘀结证症状评分、血常规指标、临床疗效以及血清IL-6与TNF-α水平。结果治疗后,两组湿热瘀阻证症状评分明显降低,且治疗组明显低于对照组,差异有统计学意义(P<0.01);治疗后,两组白细胞和中性粒细胞计数明显降低,且治疗组明显低于对照组,差异有统计学意义(P<0.01);治疗组的总有效率明显高于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清IL-6与TNF-α水平明显降低,差异有统计学意义(P<0.01),且治疗组明显低于对照组,差异有统计学意义(P<0.01)。结论黄芩牡丹汤加味保留灌肠联合抗生素治疗盆腔炎性疾病后遗症的效果良好,能改善患者的症状体征,抑制炎症,提高治疗效果。
Objective To investigate the efficacy of retention enema of Huangqin Mudan Decoction(黄芩牡丹汤)combined with antibiotics in treating sequelae of pelvic inflammatory disease and its effects of serum levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α).Methods Ninety-two sequelae of pelvic inflammatory diseases female patients admitted to the hospital from January 2018 to January 2020 were selected according to random number table method and were divided into treatment group(46 cases)and control group(46 cases).Two groups were given the routine antibiotic treatment measures.The control group was given nude purple bead tablet,and the treatment group was treated with retention enema of Huangqin Decoction.Both groups were treated for 3 months.Symptom scores,blood routine index,clinical efficacy and serum levels of IL-6 and TNF-α were compared between the two groups.Results After treatment,the symptom scores of syndrome of dampness heat and blood stasis accumulation in the two groups were significantly decreased,and the scores of the treatment group were significantly lower than those of the control group(P<0.01).After treatment,the counts of leukocytes and neutrophils in the two groups were significantly decreased,and the counts in the treatment group were significantly lower than those in the control group(P<0.01).The total effective rate of the treatment group was significantly higher than that of the control group(P<0.05).After treatment,the serum levels of IL-6 and TNF-α in the two groups were significantly decreased(P<0.01),and the levels in the treatment group were significantly lower than those in the control group(P<0.01).Conclusion The efficacy of retention enema of Huangqin Mudan Decoction combined with antibiotics in the treatment of sequelae of pelvic inflammatory disease is significant and can improve the symptoms and signs of patients,inhibit the inflammation and increase the therapeutic effect.
