摘要
目的探讨独活寄生汤结合雷火灸治疗膝骨性关节炎(knee osteoarthritis,KOA)风寒湿痹证的疗效及对炎症反应和氧化应激的影响。方法选取2020年11月—2021年10月在河南中医药大学第一附属医院治疗的KOA患者96例,随机分组为对照组和观察组各48例;对照组予以塞来昔布胶囊口服(0.2 g/次,1次/d),观察组予以独活寄生汤(每日1剂)口服结合雷火灸(炙神阙、关元);两组均连续治疗4周。比较两组临床疗效,于治疗前和治疗后行疼痛视觉模拟评分(VAS)、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)、Lequesne指数评分的测评,并行血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、血沉(ESR)、丙二醛(MDA)、超氧化物歧化酶(SOD)的测定。结果相比对照组,观察组治疗总有效率(90.91%,40/44)显著提高(P<0.05)。治疗后,观察组VAS评分、WOMAC评分和Lequesne指数评分均显著低于对照组(P<0.05)。治疗后,观察组血清CRP、TNF-α、ESR和MDA水平均显著低于对照组(P<0.05),SOD水平显著高于对照组(P<0.05)。结论独活寄生汤结合雷火灸治疗KOA风寒湿痹证疗效显著,能够有效缓解疼痛,改善膝关节功能,其作用机制可能降低炎症状态和改善氧化应激有关。
Objective To investigate the curative effect of Duhuo Jisheng Decoction(独活寄生汤)combined with thunder-fire moxibustion in the treatment of knee osteoarthritis(KOA)with wind-cold and dampness Bi syndrome and its effect on inflammatory response and oxidative stress.Methods A total of 96 KOA patients treated in the First Affiliated Hospital of Henan University of Chinese Medicine from 2020/11 to 2021/10 were randomly divided into control group(48 cases)and observation group(48 cases).The control group was given celecoxib capsules orally(0.2 g/time,1 time/d),and the observation group was given Duhuo Jisheng Decoction(1 dose daily)orally combined with thunder-fire moxibustion at Shenque(RN8)and Guanyuan(RN4).both groups were treated for 4 weeks.The clinical efficacy was compared between the two groups.Visual analogue scale(VAS),Western Ontario and McMaster University Osteoarthritis Index(WOMAC)and lequesne index score were evaluated before and after treatment.Serum C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),erythrocyte sedimentation rate(ESR),malondialdehyde(MDA)and superoxide dismutase(SOD)were measured.Results Compared with that of the control group,the total effective rate of the observation group(90.91%,40/44)was significantly improved(P<0.05).After treatment,the VAS score,WOMAC score and lequesne index score of the observation group were significantly lower than those of the control group(P<0.05).After treatment,the serum levels of CRP,TNF-α,ESR and MDA in the observation group were significantly lower than those in the control group(P<0.05),and the SOD level was significantly higher than that in the control group(P<0.05).Conclusion Duhuo Jisheng Decoction combined with thunder-fire moxibustion has obvious therapeutic effect on KOA of wind-cold and dampness Bi syndrome and can effectively relieve the pain and improve the knee joint function.Its mechanism may be related to reducing inflammation and improving oxidative stress.
