摘要
目的研究分析活血消癥杀胚汤剂联合宫腔镜电切术治疗剖宫产瘢瘢妊娠的临床疗效。方法将该院2020年1月—2021年12月收治的100例I型剖宫产瘢痕妊娠(CSP)的患者按随机数字法分为两组,治疗组51例,采用活血消癥杀胚汤剂联合宫腔镜电切术进行治疗。对照组49例,采用常规超声监视下刮宫术。随机回访3个月,对比两组患者术后疗效。结果治疗组患者的临床疗效优于对照组患者,差异具有统计学意义(P<0.05);治疗组患者血人绒毛膜促性腺激素(β-hCG)恢复正常时间和手术耗时、术中出血量、术后阴道流血天数、住院总天数、优于对照组,且差异具有统计学意义(P<0.05);治疗组术后半月、1个月、2个月、随访复查β-HCG均低于对照组,差异具有统计学意义(P<0.05)。3个月随访复查人绒毛膜促性腺激素(human chorionic gonadotrophin,HCG)两组患者均已下降至正常值;对比两组患者不良反应发生率,治疗组(3.9%,2/51)低于对照组(12.2%,8/49),具有统计学意义(P<0.05)。结论在CSP患者治疗过程中,采用活血消癥杀胚汤剂联合宫腔镜电切术效果显著,缩短患者手术时间,恢复时间,值得推广应用。
Objective To observe the clinical effect of Huoxue Xiaozheng Shapei Decoction(活血消癥杀胚汤)and hysteroscopic electric resection in the treatment of cesarean scar pregnancy.Methods In the hospital from January 2020 to December 2021,100 patients of type I cesarean scar pregnancy(CSP)were divided into two groups,including treatment group 51 cases which were treated with Huoxue Xiaozheng Shapei Decoction combined with hysteroscopic electric resection and the control group 49 cases which were treated with conventional ultrasound monitoring curettage.The random visit lasted for 6 months and the effects of the two groups were compared.Results There was no significant difference between all basic data in the two groups(P<0.05).The patients in the treatment group had better clinical efficacy than those in the control group.The difference was statistically significant(P<0.05).The normal recovery of human chorionic gonadotropi(β-hCG)and time consuming,intraoperative bleeding,postoperative vaginal bleeding and total days of hospitalization were better than those of the control group and the difference was statistically significant(P<0.05).In the treatment group,half a month,one and two months after operation and during the follow-up period,the levels ofβ-hCG were lower than those in the control group.The difference was statistically significant(P<0.05).After 3 months follow-up,HCG levels in both groups were decreased to normal levels.The incidence of adverse effects in the treatment group(3.9%,2/51)was lower than that of the control group(12.2%,8/49)with statistically significant difference(P<0.05).Conclusion In the treatment of CSP patients,Huoxue Xiaozheng Shapei Decoction combined with hysteroscopic electric resection can shorten the operation time and recovery time,worthy of promotion.
