慢性阻塞性肺疾病患者肺部感染病原菌及血清miR-124-3p、PSGL-1、HPT的诊断效能 被引量:1

摘要 目的 研究慢性阻塞性肺疾病患者肺部感染病原菌分布及血清微小核糖核酸124-3p(miR-124-3p)、P-选择素糖蛋白配体-1(PSGL-1)、触珠蛋白(HPT)的诊断效能。方法 回顾性收集2020年1月-2022年7月医院收治的144例慢性阻塞性肺疾病患者临床资料,根据患者是否发生肺部感染将其分为感染组(42例)和非感染组(102例),统计慢性阻塞性肺疾病肺部感染患者病原菌分布情况及两组一般资料,比较两组血清miR-124-3p、PSGL-1、HPT水平,采用受试者工作特征(ROC)曲线分析血清miR-124-3p、PSGL-1、HPT对慢性阻塞性肺疾病患者肺部感染的诊断效能。结果 144例慢性阻塞性肺疾病患者发生肺部感染患者42例,共分离出病原菌51株,其中革兰阴性菌40株,占比78.43%;革兰阳性菌5株,占比9.80%;真菌6株,占比11.76%;感染组血清miR-124-3p水平低于非感染组,血清PSGL-1、HPT水平高于非感染组(P<0.05);miR-124-3p、PSGL-1、HPT联合检测曲线下面积(AUC)值高于miR-124-3p、PSGL-1、HPT(P<0.05);miR-124-3p敏感度高于PSGL-1、HPT,HPT及联合检测敏感度高于PSGL-1(P<0.05);PSGL-1、HPT及联合检测特异度高于miR-124-3p(P<0.05)。结论 慢性阻塞性肺疾病患者肺部感染病原菌主要种类为鲍氏不动杆菌、铜绿假单胞菌、金黄色葡萄球菌、白假丝酵母,同时血清miR-124-3p、PSGL-1、HPT联合对慢性阻塞性肺疾病患者肺部感染的诊断效能较高。 OBJECTIVE To investigate the distribution of pathogenic bacteria in pulmonary infection in patients with chronic obstructive pulmonary disease and to evaluate the diagnostic efficacy of serum micrornas 124-3p(miR-124-3p),P-selectin glycoprotein ligand 1(PSGL-1)and thiosglobin(HPT).METHODS Clinical data of 144 patients with chronic obstructive pulmonary disease admitted to the hospital from Jan 2020 to Jul 2022 were retrospectively collected.According to whether pulmonary infection occurred,the patients were divided into the infected group(42 cases)and the non-infected group(102 cases).The distribution of pathogenic bacteria and the general data in patients with pulmonary infection with chronic obstructive pulmonary disease were analyzed.Serum levels of miR-124-3p,PSGL-l and HPT between the two groups were compared,and the diagnostic efficacy of serum miR-124-3p,PSGL-1 and HPT in pulmonary infection in patients with chronic obstructive pulmonary disease was analyzed by receiver operating characteristic curve(ROC).RESULTS Pulmonary infection occurred in 42 of 144 patients with chronic obstructive pulmonary disease.A total of 51 strains of pathogenic bacteria were isolated,including 40 strains of gram-negative bacteria,accounting for 78.43%,5 strains of gram-positive bacteria,accounting for 9.80%and 6 strains of fungi,accounting for 11.76%.The levels of serum miR-124-3p in the infected group were lower than those in the non-infected group,and the levels of serum PSGL-l and HPT were higher than those in the non-infected group(P<0.05).The area under curve(AUC)value of combined detection of miR-124-3p,PSGL-1,HPT were higher than that of individual detection(P<0.05).The sensitivity of miR-124-3p was higher than that of PSGL-1 and HPT,while the sensitivity of HPT and combined detection was higher than that of PSGL-1(P<0.05).The specificity of PSGL-1,HPT and combined detection was higher than that of miR-124-3p(P<O.05).CONCLUSION The main types of pulmonary infection pathogens in patients with chronic obstructive pulmonar
